Requirements
  • A high school diploma or General Educational Development (GED) credential is required.
  • Meet Center for Medicare and Medicaid Services-approved state and/or national certification requirements within the required state or CMS timeline.
  • Hold all appropriate state licensure, education, and training required for the role.
  • Complete the organization dialysis training program successfully as a condition of continued employment.
  • Maintain current cardiopulmonary resuscitation (CPR) certification successfully as a condition of continued employment.
  • Previous patient care experience in a hospital or related facility is preferred.
  • Pass the Ishihara color blindness test as a condition of employment; failure does not preclude employment if reasonable accommodation can be made.
  • Perform direct patient care involving heavy lifting, moving patients, assisting with ambulation, prolonged standing, bending, and lifting solutions and equipment within the specified weight limits.
  • Travel between assigned facilities and various community locations, with possible travel to regional, business unit, and corporate meetings.
Responsibilities
  • Provide safe and effective dialysis therapy under the direct supervision of a licensed nurse and in accordance with organizational policies, procedures, training, and applicable regulations.
  • Set up and operate hemodialysis machines.
  • Assist the healthcare team with patient-specific education about adequacy measures, disease processes, and vascular access.
  • Welcome assigned patients and inquire about their wellbeing since their last treatment.
  • Evaluate vascular access patency, perform vascular access cannulation, and administer heparin as delegated or permitted by state law.
  • Obtain and document pre- and post-treatment vital signs, weights, and vascular access evaluations.
  • Initiate dialysis treatments according to prescribed orders, including blood flow and dialysate flow.
  • Evaluate intradialytic problems and provide interventions according to physician orders or direction.
  • Monitor patient responses to dialysis therapy and report significant information or changes in condition to the registered nurse or supervisor.
  • Discontinue dialysis treatments according to established procedures, evaluate patients before termination of venous access, obtain hemostasis, and apply appropriate dressings.
  • Evaluate patients before discharge.
  • Calculate and enter dialysis machine programming for ultrafiltration goals, treatment time, and ultrafiltration modeling as prescribed.
  • Ensure appropriate safety alarms are enabled, including Narrow Venous Limits 160.
  • Record vital signs, treatment parameters, treatment evaluations, and other patient treatment information accurately and promptly on the hemodialysis treatment sheet and progress notes when applicable.
  • Observe patients and conduct machine safety checks according to facility policy, reporting unusual findings to the registered nurse or supervisor.
  • Perform and document interventions for unusual patient status and document patients' responses.
  • Evaluate vascular access during treatment, including arterial and venous monitoring pressures, and provide and document interventions as needed.
  • Obtain prescribed laboratory testing and prepare specimens for collection.
  • Clean blood spills immediately with appropriate disinfectant according to facility policy.
  • Prepare, organize, and use supplies and equipment efficiently to prevent wastage.
  • Monitor patients performing self-care under registered nurse supervision.
  • Assist with training applicable direct patient care staff on appropriate programs under nurse supervision.
  • Follow organizational business policies, procedures, systems, and ethical business practices.
  • Maintain environmental integrity and aesthetics by keeping all areas safe and clean.
  • Clean and disinfect dialysis machines, chairs, equipment, and surrounding areas between treatments according to facility policy.
  • Prepare for dialysis treatments, complete required pretreatment dialysis machine alarm testing including Pressure Holding Test, and document as appropriate.
  • Initiate the Solution Delivery System.
  • Participate in Continuous Quality Improvement activities and document participation.
  • Enter all treatment data accurately and promptly into the designated clinical application.
  • Review treatment sheets for completeness, ensure nursing signatures are documented, and arrange correction of omitted entries.
  • Prepare initial patient charts for admission and compile paperwork for placement in the chart.
  • Prepare and print laboratory requisitions for prescribed or stat specimens according to laboratory destination.
  • Collect, label, prepare, and store laboratory samples according to laboratory specifications and ensure collection by the appropriate laboratory courier.
  • Assist with collecting information for infection control audits.
  • Support staff and patient adherence to infection control practices and follow infection control policies and procedures.
  • Participate in and reinforce infection control education for patients and families under the direction of the clinical manager or designee as permitted by state law.
  • Follow newly admitted patients through their first four weeks of dialysis and coordinate transitional dialysis care.
  • Reinforce education and care for new patients as permitted by state law.
  • Liaise with appropriate facility staff to provide the best patient experience and ensure disciplines are scheduled according to the care plan.
  • Assist patients in understanding home dialysis products, the benefits of home dialysis, and how home dialysis can work for them.
  • Set up and perform treatments on all machines used in the transitional care unit.
  • Assist patients in understanding treatment options, including demonstrating peritoneal dialysis setup.
  • Assist with coordinating transitions to the patient's chosen dialysis modality.
  • Assist with supply ordering and inventory for the transitional care unit.
