  • Complete two hours of stroke-related continuing education contact hours annually.

Responsibilities
  • Interpret, evaluate, and accurately process drug orders for patients in assigned areas; use pharmacy operational computer systems and integrated hospital systems; follow policies and procedures to maintain accurate patient medication profiles; initiate appropriate medication therapy across neonatal, pediatric, adolescent, adult, and geriatric populations; and individualize therapy based on medication factors and patients’ physical and psychological needs.
  • Identify and resolve problems with medication orders or therapeutic regimens; clarify issues with prescribers; and suggest order adjustments or alternative medication therapies to prevent adverse events or ensure adherence to institutional policies.
  • Provide concise, applicable, comprehensive, and timely drug-information responses to patients and healthcare providers; supervise and coordinate pharmacy technician work processes as appropriate for patient care assignments; ensure proper drug utilization in accordance with formulary criteria and pharmacy policies; and support clinical pathways and quality assurance programs.
  • Communicate between shifts and with peers to ensure continuity of care and optimal pharmaceutical services; participate in management of medical emergencies and respond to adult and pediatric codes; and lead medication-management processes as a member of the Code Blue team.
  • Take responsibility for patient welfare by performing necessary aspects of the medication-use system, ensuring comprehensive and individualized pharmaceutical care, and overseeing clinical and operational workflow within assigned patient-care areas.
  • Provide clinical and direct patient-care services for assigned populations; respond to consultation for pharmacy interventions and make evidence-based, patient-centered medication-therapy recommendations; collaborate with healthcare providers as an active multidisciplinary team member; and attend designated multidisciplinary work or care rounds as appropriate.
  • Ensure accurate and timely communication of patient-specific medication information to those who need it and document information in the patient’s medical record when requested or directed by policy.
  • Document pharmaceutical-care interventions in the departmental reporting system; identify, mitigate, prevent, and report potential and actual medication errors or adverse drug events; recommend medication-system changes to prevent future problems; and implement, evaluate, and redesign evidence-based pharmaceutical-care plans based on patient progress and evolving goals.
  • Establish collaborative pharmacist-patient relationships; educate patients on safe and effective medication use, possible side effects, and the role of prescribed medications in the overall therapeutic-care plan; and provide discharge medication review and counseling, including medication reconciliation, as appropriate.
  • Identify opportunities to improve medication use to advance patient safety, maximize therapeutic outcomes, or control costs; participate in planning, implementing, and evaluating programs; and maintain current evidence-based knowledge of drugs and drug therapy through relevant professional journals and readings.
  • Support peers and other practice areas as needed, in addition to daily assignments, as part of a team approach to patient care within the Pharmacy Department.
  • Serve as an internal and external liaison for the goals and vision of Pharmacy Department services.
  • Represent the Pharmacy Department at hospital meetings and events as needed and participate actively in the Pharmacy Committee and other committees or task forces as requested.
  • Precept pharmacy residents and serve as a member of the Residency Advisory Committee; precept student experiential education programs; and maintain training and experience appropriate to assigned pharmacy-practice areas.
  • Design and implement quality-improvement changes to the organization’s medication-use system; conduct practice-related projects, clinical research, or pharmacy CQI initiatives using project-management skills; and review protocols, orders, and policies as requested.
  • Provide evidence-based medication and practice-related education to healthcare professionals and department members; review, evaluate, and discuss pharmacy-practice investigation results; provide updates on therapeutics, policies and procedures, formulary, or regulatory issues; and participate in creating and disseminating pharmacy newsletters for nursing, medical, and ancillary staff.
  • Participate in orientation, education, and mentoring of new staff, including pharmacists, pharmacy residents, nurses, and pharmacy technicians, as needed.
  • Contribute to pharmacy-practice knowledge through presentations, publications, and professional-affiliation activities; maintain a professional profile and active membership in relevant pharmacy or medical organizations; and join local, state, or national professional organizations.
  • Perform other duties as assigned.

Capital Health begins construction of first micro-hospital in Bucks County. Thursday, September 10, 2026 To help meet the growing demand for more robust health care services in Bucks County, Capital Health has begun construction of Capital Health - Oxford Valley, the first micro-hospital in Bucks County. Micro-hospitals are an emerging option in health care, with fully accredited facilities that are smaller than full-service hospitals but more comprehensive than urgent care centers or standalone emergency rooms. Micro-hospitals like Capital Health - Oxford Valley provide inpatient and emergency care 24/7 with services designed to meet the needs of their home communities. Conveniently located near the Oxford Valley Mall, the new facility at 280 Middletown Boulevard in Langhorne, Pennsylvania will offer a range of services to nearby communities, including an emergency department, diagnostic procedures and services, and some inpatient beds for short-stay, low-acuity patients. Capital Health - Oxford Valley is expected to open in early 2028. Kim Watson joins NJ PACE Association Board.

Kim Watson joins NJ PACE Association Board. The New Jersey PACE Association is pleased to welcome Kim Watson the newly appointed Executive Director of Capital Health LIFE, to the Board. Ms. Watson is a registered nurse and previously served as director of operations for Capital Health's Institute for Urban Care (IUC). During her 21 years of service at Capital Health, Kim has shared her experience in critical care and program development. She is also certified in trauma care. Kim has successfully designed, developed, and implemented programs focusing on three key areas: chronic disease, maternal and child health, and behavioral health/substance use. With a strong background in operational management, strategic planning, and community engagement, Kim has played a critical role in enhancing the health and well-being of the Trenton community. Over her tenure, Kim has served on numerous committees, including the Advisory Board of the New Jersey Cohort Advisory Committee and the Mercer County Advisory Board for the Reduction of Gun Violence. Kim completed her undergraduate studies at Post University in Waterbury, Connecticut, where she also received her master's degree with a specialization in nursing management and organizational leadership. She received an associate degree in liberal arts and sciences from Mercer County Community College in Trenton, New Jersey and a Diploma of Nursing from Capital Health School of Nursing, also in Trenton. Thursday, August 13, 2026 Capital Health Regional Medical Center in Trenton, New Jersey and Capital Health Medical Center - Hopewell recently received the American Heart Association's Gold Plus Get with The Guidelines(R)- Stroke Quality Achievement Award. This award recognizes Capital Health's commitment and success in ensuring stroke patients receive the most appropriate treatment according to nationally recognized, research-based guidelines based on the latest scientific evidence. As one of 12 state-designated comprehensive stroke centers in New Jersey and home to Capital Institute for Neurosciences (for patients who require the most advanced treatments for neurovascular and stroke care), Capital Health Regional Medical Center received the American Heart Association's Target: Stroke(SM) Honor Roll Elite Plus award and the Target: Stroke(SM) Honor Roll Advanced Therapy award. To qualify for these recognitions, Capital Health Regional Medical Center must meet quality measures developed to reduce the time between a patient's arrival at the hospital and treatment with clot-busting medication. Capital Health Medical Center - Hopewell received the American Heart Association's Target: Stroke(SM) Honor Roll Elite award. Additionally, Capital Health Regional Medical Center and Capital Health Medical Center - Hopewell each received the Association's Target: Type 2 Diabetes Honor Roll award. Hospitals like Capital Health Regional Medical Center and Capital Health Medical Center - Hopewell earn these awards by demonstrating a commitment to providing quality care and meeting specific quality achievement measures for the diagnosis and treatment of stroke patients at a set level for a designated period. These measures include evaluation of the proper use of medications and other stroke treatments aligned with the most up-to-date, evidence-based guidelines with the goal of speeding recovery and reducing death and disability for stroke patients. Before discharge, staff provide patients with education on managing their health, as well as information to make their follow-up appointment, including the name and contact information for the physician and when to make their appointment. Stroke is the fourth leading cause of death and a leading cause of adult disability in the U.S., according to the American Heart Association. A stroke occurs when a blood vessel that carries oxygen and nutrients to the brain is either blocked by a clot or bursts. When that happens, part of the brain cannot get the blood and oxygen it needs, so brain cells die. Early stroke detection and treatment are key to improving survival, minimizing disability and supporting faster recovery. Get With the Guidelines-Stroke was developed to assist health care professionals to provide the most up-to-date, research-based guidelines for treating stroke patients. Each year, program participants apply for the award recognition by demonstrating how their organization has committed to providing quality care for stroke patients. Capital Health launches program to provide hospital-level care at patients' homes.

Capital Health launches program to provide hospital-level care at patients' homes. Capital Health has launched a new program that delivers hospital-level care to patients in their own homes, according to an Aug. 12 announcement. The Hospital at Home Program is designed to manage capacity at Capital Health hospitals by allowing eligible patients to receive care outside the traditional hospital setting. The program offers personalized treatment through daily virtual visits with a doctor and in-person nurse check-ins, as well as services such as physical therapy, lab work, and nutrition support. These services are coordinated by a multidisciplinary team. Participants must be New Jersey residents aged 18 or older, live within a 30-minute drive of Capital Health Medical Center - Hopewell, and meet specific clinical and safety criteria for eligibility. "The innovative care model for this program allows eligible patients to receive hospital-level diagnostics, treatments, and daily clinician visits in the comfort of their own homes," said Al Maghazehe, president and CEO of Capital Health. "Supported by our clinical teams and enhanced remote monitoring technology, Hospital at Home isn't just transforming how we provide care. More importantly, it means better outcomes for patients with fewer complications, faster recovery, and lower overall costs of care." Patients who may qualify include those with conditions such as pneumonia, chronic obstructive pulmonary disease (COPD), congestive heart failure (CHF), cellulitis, or other acute but stable diagnoses that can be managed safely at home. After diagnosis and initial treatment at Capital Health Medical Center - Hopewell, interested patients are assessed by a clinical review team before being admitted into the home-based program. Participants receive daily video visits from physicians affiliated with Capital Health Medical Group and twice-daily in-person nurse visits. The care team also includes pharmacists, therapists, nutritionists, social workers, case managers, and other specialists who help coordinate all aspects of patient support. To ensure safety and connectivity between patients and providers around the clock, each participant receives advanced equipment such as tablets for video calls, direct-dial phones for urgent needs, blood pressure cuffs, scales for weight monitoring, oxygen monitors or blood glucose devices when needed - all set up by the Hospital at Home team with detailed instructions provided.
