- At least one year of Licensed Practical Nurse experience in an acute care setting.
- A valid Connecticut Licensed Practical Nurse license.
- Basic Life Support certification.
- A physical examination completed within the past year.
- A PPD or QuantiFERON test completed within the past year, or a chest X-ray completed within the past five years.
- Authorization to work in the United States.
- Provide high-quality, specialized one-on-one home care.
- Work 12-hour day or overnight shifts and per-visit assignments across New Haven County.
- Experience with home care.
- Experience with tracheostomy and ventilator care.
- A driver's license is preferred.
Affirmed Home Care is a private home-care provider serving the New York metropolitan region.
