Requirements
  • A specialized diploma from an accredited school of nursing is required.
  • Maintain a current registered nursing license in the state of practice.
  • Demonstrate knowledge of patient- and family-centered practice, including conveying respect and collaborating with patients and designated care partners in care planning.
  • Demonstrate cultural awareness and the ability to connect respectfully with people from diverse backgrounds.
  • Apply effective communication concepts, tools, and techniques to transmit, receive, and accurately interpret ideas, information, and needs.
  • Maintain confidentiality, adhere to HIPAA, demonstrate ethical conduct, manage time effectively, and demonstrate willingness to precept nursing students.
  • Apply nursing and medical terminology and assess patient ailments and injuries for appropriate triage.
  • Use child and adult education techniques and address health-literacy barriers when educating patients and designated care partners about conditions, treatments, medications, and recovery steps.
  • Apply pharmacology knowledge in primary care, including medications and preventive immunizations, and use pharmacology resources for professional and patient education.
  • Use patient-assessment and nursing-diagnosis techniques to analyze symptoms and triage patients effectively.
  • Document medical histories, symptoms, vital signs, medications, diagnoses, treatment plans, procedures, tests, medical results, and nursing care in written or electronic form.
  • Attend and successfully complete electronic medical record training and remain proficient in the electronic medical record and its tasking function.
  • Maintain Basic Life Support certification from the American Heart Association.
  • Hold a Pennsylvania registered nurse license.
  • Meet the physical demands of lifting and carrying 40–50 pounds, frequent to continuous standing and walking, patient transport, kneeling, bending, squatting, and performing cardiopulmonary resuscitation.
Responsibilities
  • Use the nursing process to address the physical, psychological, emotional, and social needs of ambulatory patients.
  • Collaborate with clinic providers and support staff to deliver clinical nursing interventions and coordinated patient care.
  • Integrate evidence-based practice, clinical expertise, patient and family preferences, and cultural beliefs and values in healthcare delivery.
  • Collaborate with care providers regarding patient care and management and support communication between primary care providers and specialists.
  • Support implementation of clinical care pathways for common conditions and participate in quality-improvement opportunities.
  • Provide evidence-based ambulatory nursing interventions during clinical visits, telephone encounters, and electronic communications.
  • Educate patients and designated care partners about preventive care, chronic disease management, medication management, and common illnesses treated in primary care.
  • Document patient and designated care partner interactions and clinical care in the electronic medical record according to organizational standards and applicable regulations.
  • Coordinate care for abnormal results, triage, registered nurse verification of vaccinations and injections, electronic in-baskets, prior authorizations, and forms.
  • Oversee clinical care supporting the assessment, promotion, maintenance, and restoration of patient health and wellness.
  • Minimize patient and care-partner risk by following established nursing practices and protocols.
  • Use information technology to communicate, manage knowledge, and mitigate errors.
  • Use data to monitor care-process outcomes and support ethical decision-making.
  • Use improvement methods to design and test changes that improve patient-care quality and safety.
  • Function within ambulatory interprofessional teams using shared decision-making to achieve quality patient care.
  • Provide safe, compassionate, and coordinated care based on patient preferences, priorities, needs, and cultural beliefs and values.
  • Create an environment of belonging and inclusion and apply problem-solving and conflict-resolution skills.
  • Participate actively in team huddles and assemblies to provide announcements and clinical-operations updates.
  • Give and receive feedback and integrate appropriate changes into practice.
  • Pursue continued professional growth and development.
Desired Qualifications
  • A Bachelor of Science in Nursing, completion of a Bachelor of Science in Nursing, or an appropriate advanced degree within three years of hire is strongly preferred.
  • One year of nursing experience in an ambulatory setting is preferred.
  • Obtain ambulatory care nursing certification.
  • Participate in or support quality, evidence-based practice, research, and nursing-governance initiatives.
