- A diploma or Associate’s Degree in Nursing is required.
- A New York State Registered Nurse license is required.
- A valid New York State driver’s license is required.
- The role requires frequent driving, repeatedly entering and exiting vehicles, and occasionally lifting and carrying equipment weighing up to 100 pounds or more.
- The role requires strong fine and gross motor control, firm grasping, and heightened visual and auditory awareness.
- The role requires the ability to make rapid decisions and independently manage patient care in field or home settings.
- Flexibility for extended or on-call schedules is typically required.
- Provide comprehensive assessments of the biopsychosocial needs of acutely ill clients in their homes.
- Perform skilled nursing care and prescribed treatments based on physician orders.
- Provide client and family education in accordance with the client assessment and plan of care.
- Develop and document individualized care plans tailored to each patient’s needs, with interdisciplinary health team support as needed.
- Communicate patient health status, treatment plans, and progress effectively; regularly communicate with office and supervisory staff.
- Coordinate client services and referrals efficiently and in a timely manner.
- Accurately and completely document client care.
- Document and implement physician orders in accordance with Department of Health and program standards.
- Adhere to department and system protocols, local, state, and federal regulations, and education requirements.
- Two years of home care experience is preferred.
- Comprehensive nursing assessment and documentation skills, decisive judgment, and professional behavior are preferred.
- Bilingual ability is preferred.
