About the job

Requirements
  • At least 6 years of software or platform engineering experience, including at least 4 years operating machine learning systems in production, or equivalent depth.
  • Strong experience in machine learning engineering, including training pipelines, evaluation, deployment patterns, monitoring, and iteration loops.
  • Strong Python engineering skills and production-grade experience building APIs and services.
  • Hands-on experience with large language model systems in production and machine learning engineering, including training pipelines, evaluation, deployment patterns, monitoring, and iteration loops.
  • Strong experience with Google Cloud Platform services and cloud-native patterns.
  • Experience with Vertex AI (pipelines, endpoints, feature store, model registry, evaluation) and/or managed vector search on Google Cloud Platform.
  • Experience with containerization and orchestration, including Docker, Kubernetes/Google Kubernetes Engine, and/or Cloud Run.
Responsibilities
  • Design and operate machine learning platforms supporting end-to-end workflows for data ingestion, feature engineering, training, evaluation, deployment, and monitoring.
  • Build and maintain continuous integration and continuous delivery for machine learning, including testing, packaging, versioning, reproducibility, automated rollbacks, and approvals.
  • Implement machine learning operations best practices, including model registry, experiment tracking, lineage, governance, and reproducible training environments.
  • Develop scalable training infrastructure, including distributed training, GPU scheduling, cost controls, and auto-scaling.
  • Create and maintain feature pipelines and feature stores, ensuring consistency between training and inference and preventing training-serving skew.
  • Establish model monitoring and observability for performance, drift, bias and fairness signals where relevant, latency, throughput, and data quality.
  • Build and own end-to-end large language model delivery pipelines for prompt versioning, retrieval, orchestration, evaluation, deployment, monitoring, and iterative improvement.
  • Create large language model evaluation harnesses for offline and online evaluation, including golden datasets, automated regression testing, human-in-the-loop review workflows, and risk scoring.
  • Build cost controls, including token and cost budgeting, caching strategies, auto-scaling, and performance tuning.
  • Productionize machine learning models on Google Cloud Platform using containers and orchestration such as Google Kubernetes Engine or Cloud Run, and build continuous integration and delivery for machine learning and large language model systems with automated tests and safe rollouts.
  • Implement observability through tracing, metrics, logs, dashboards, and alerting for model and system health, including latency, token usage, error rates, retrieval quality, hallucination indicators, and drift where relevant.
  • Design systems with security and privacy by default, including identity and access management, least privilege, secrets management, audit logs, encryption, data retention, and protected health information and personally identifiable information handling.
  • Implement governance through model and prompt lineage, dataset provenance, evaluation traceability, and approval workflows aligned with healthcare compliance expectations.
  • Integrate guardrails, including content filters, policy checks, prompt injection defenses, structured output validation, and fallback strategies.

About the company

C the Signs uses artificial intelligence to help healthcare professionals detect cancer earlier by analyzing patient symptoms, clinical data, and risk factors in real time to suggest diagnostic and referral pathways. It integrates into primary care and broader health system workflows, translating complex cancer guidelines into practical decisions across all cancer types, including rare ones. The platform supports faster referrals, reduces diagnostic delays, and enables more personalized cancer care while focusing on population health to identify disparities and align with national targets. Its goal is to improve patient outcomes by enabling earlier diagnosis and scalable, data-driven cancer care across health systems.

What believers are saying

  • March 2026 North East Essex launched AI case-finding for oesophageal and stomach cancers.
  • November 2025 studies showed 808% NHS ROI and 12% higher cancer detection.
  • April 2025 research across 1,084 GP practices found 20% more cancers, fewer urgent referrals.

What critics are saying

  • NHS procurement concentration leaves revenue exposed if ICB rollouts stall in 2026.
  • US expansion after January 2025 Khosla funding faces entrenched EHR competitors and sales cycles.
  • False negatives or adverse regulatory findings would destroy clinician trust and halt deployments.

What makes C the Signs unique

  • Founded by NHS doctors, C the Signs embeds directly into GP workflows.
  • By March 2026, it supported 1,500-plus GP practices and 11,000 clinicians.
  • Its AI uses real-world evidence to triage 100-plus cancer types within 60 seconds.

NHS launches first AI-driven early detection pathway for oesophageal and stomach cancers

A UK-first, AI-enabled pathway has launched in North East Essex to improve early detection of oesophageal and stomach cancers, which are among England's hardest cancers to diagnose early. Around four in five cases are currently diagnosed at advanced stages. Delivered by AstraZeneca, C the Signs, NHS Suffolk and North East Essex ICB, Health Innovation East and GP Primary Choice, the programme uses AI to analyse electronic health records and identify at-risk patients. Eligible patients receive SMS invitations to complete digital assessments, with high-risk cases triaged to appropriate diagnostics including capsule sponge testing or endoscopy. C the Signs already supports over 1,500 GP practices across England. The programme aligns with the National Cancer Plan's ambition to shift from reactive to predictive, risk-based approaches for cancers without established screening programmes.

C the Signs Raises $8M for U.S. Expansion

C the Signs, an AI-powered cancer detection company, raised $8M in a funding round led by Khosla Ventures to expand from the UK to the US. The company's platform analyzes EHRs to identify cancer risk early, improving survival rates. In the UK, it identified 20.7% of breast cancer cases up to five years earlier and improved early-stage ovarian cancer diagnosis by 53.3%. The funding aims to enhance early detection and personalized care in the US healthcare system.
