  • Must be a registered nurse with a Bachelor of Science in Nursing or Master of Science in Nursing.
  • Must have at least five years of recent registered nurse bedside experience in an acute hospital setting.
  • Must have critical care or intensive care unit nursing experience to be considered.
  • Must have a Bachelor of Nursing degree.
  • Must hold a current State of Texas nursing license or a temporary or compact license to practice professional nursing.
  • Must have five years of inpatient acute-care bedside experience on a nursing unit.
  • Must demonstrate observation, analytical thinking, problem-solving, verbal and written communication, organization, planning, computer usage, independent work, adaptability, flexibility, and self-motivation.
  • Must possess working knowledge of adult medicine and be able to assess, evaluate, and teach.
  • Must comply with HIPAA and Code of Conduct policies.
  • Must meet mandatory or continuing education and skills-competency requirements.
Responsibilities
  • Review inpatient medical records concurrently to identify opportunities to improve physician documentation.
  • Facilitate clinical-documentation modifications through concurrent interaction with physicians and healthcare team members.
  • Communicate with physicians, case managers, coders, and other healthcare team members to facilitate comprehensive inpatient medical-record documentation.
  • Use the hospital’s designated clinical documentation system to identify opportunities related to physician and hospital outcomes.
  • Provide or coordinate education for internal customers on compliance, coding, and clinical-documentation issues.
  • Act as a consultant to coders when additional information or documentation is needed to assign the correct diagnosis-related group.
  • Evaluate medical-staff and healthcare-team documentation daily in accordance with the designated clinical documentation system.
  • Gather and analyze information related to documentation findings and outcomes.
  • Develop physician-education strategies to promote complete and accurate clinical documentation and correct negative trends.
  • Identify patterns, trends, variances, and opportunities to improve documentation review and processes.
  • Research literature and remain current on Centers for Medicare & Medicaid Services rules and regulations, incorporating changes into daily practice.
  • Assist with developing and reporting performance measures and prepare physician-specific data information.
  • Confer with key physicians about outcome information and documentation clarity, completeness, and correct diagnosis-related-group designation.
  • Coach physicians to improve documentation so it accurately reflects intensity of services and severity of illness.
  • Attend hospital service-line meetings, review outcome information, and educate physicians on service-line-specific improvement opportunities.
  • Document conferences and results.
  • Assist with special projects and other assigned duties.
  • Ensure safe care for patients, staff, and visitors and follow hospital policies, procedures, standards, budget, time-management, supply-management, productivity, and service-quality requirements.
  • Serve as a preceptor, mentor, and resource to less experienced staff.
  • Promote professional growth and development and support department-based goals.

  • Intensive care unit experience is preferred.
  • Knowledge of federal, state, and private payer regulations is preferred.

Memorial Hermann clinicians use Epic's AI tool to cut patient follow-up messages by 32%

Memorial Hermann Health System clinicians are using Epic's AI tool, Art, to help patients better understand their test results. The AI-powered assistant generates initial draft patient notes in clear, patient-friendly language as clinicians review lab and imaging results. Since implementation, patients have sent 32% fewer follow-up messages about test results when clinicians used Art's draft. The tool has helped Memorial Hermann's care teams write over 55,000 messages to patients, with clinicians saving an average of nine seconds per result comment. Memorial Hermann piloted the feature in December 2025 in their innovation clinic before expanding system-wide. Dr Ryan Walsh, chief medical information officer, emphasised that the technology is designed to support, not replace, clinician judgment and patient relationships.
