Requirements
  • A bachelor's degree in psychology, counseling, social work, or a related field from an accredited college or university.
  • Apply for a Counselor Agency Affiliated–Registered credential from the Washington State Department of Health within 30 days of hire.
  • Complete a state-supported WISe training module or its equivalent within 30 days of hire.
  • Complete and be able to administer Child and Adolescent Needs and Strengths (CANS) training within 30 days of hire.
  • Have an unrestricted and valid driver's license and active car insurance, and be willing to use a personal vehicle for work.
  • Know Washington Administrative Codes and statutes pertaining to the treatment and delivery of mental health services.
  • Communicate and engage effectively with individuals from varied backgrounds, trauma experiences, cultures, education levels, and socioeconomic statuses.
  • Evaluate situations, adopt effective courses of action, delegate responsibilities as appropriate, and organize and present facts effectively.
  • Establish working relationships with clinical, administrative, and support staff to develop professionalism and coordination within the team.
  • Establish and maintain effective working relationships with partner agencies, consumers, family members, and the public.
  • Communicate effectively with staff, youth, family members, administration, and the community, in person and by telephone.
  • Maintain strict confidentiality when handling consumer and family information.
  • Work under pressure and meet deadlines efficiently and consistently while balancing personal health with job requirements.
  • Travel to see clients in the community and between work sites.
  • Drive, traverse distances up to several city blocks across varied terrain, navigate stairs, and frequently enter and exit vehicles.
  • Use a computer daily and be able to lift 10–20 pounds when required.
Responsibilities
  • Provide care coordination services to clients assigned to WISe, including guiding the team process as delegated or assigned, identifying and including the client’s support network, helping clients identify and access resources, and coordinating and collaborating with community resources on clients’ behalf.
  • Encourage each team member to identify priority concerns, proactively minimize potential conflict, and acknowledge the mandates of others involved in child-serving systems.
  • Participate in developing appropriate ground rules for the team.
  • Increase natural supports in the youth’s and family’s life, and prepare for and facilitate Child and Family Team (CFT) meetings.
  • Develop meeting agendas with the youth, family, and other CFT members; schedule meetings at a location and time accommodating to the youth and family and available to all members; and prepare visual aids or tools to facilitate meetings.
  • Contact members who cannot attend meetings in advance to gather their input.
  • Develop and update the Cross-System Care Plan (CSCP) with CFT members, including Child and Adolescent Needs and Strengths (CANS) scores.
  • Provide all members with an updated care plan and documentation of progress, meeting activities, discussions, and task assignments within seven days after meetings.
  • Support implementation of CSCP plans and update or modify the plans as needed.
  • Monitor and evaluate services provided to families, contact families as needed to monitor progress, and inquire about new needs.
  • Manage the caseload in accordance with agency guidelines, risk assessment and management, and assessment of measurable progress toward agreed-upon goals and objectives.
  • Meet the agency’s productivity standards for direct service hours.
  • Maintain client records and other required WISe Cross-System Care Plan documentation, along with objectives and interventions assigned to the FSS in the child’s clinician-prepared Care Plan, in a timely manner and according to agency policies and procedures.
  • Participate in required staff meetings, regular supervision, and weekly clinical staff meetings.
  • Work a flexible schedule, including rotating 24/7 coverage with other clinical staff and adjusting work hours to meet client and family needs.
  • Perform other duties as assigned by the agency or supervisor.
