Responsibilities
  • Report to the nurse manager or nurse supervisor and meet weekly for supervision to review duties and responsibilities.
  • Manage the day-to-day operations of the nursing station and ensure workflows and policies are implemented and followed.
  • Assume nurse supervisor duties when needed and provide coverage and assistance for Patient-Centered Medical Home and other grants.
  • Supervise nursing staff, including medical assistants and medical secretaries when applicable to the site.
  • Ensure each provider has an assigned LPN or medical assistant to support the daily workflow.
  • Document all activities in the electronic medical record system after training and system implementation.
  • Coordinate and manage high-risk populations as a Patient-Centered Medical Home care-team member, lead daily huddles, and communicate with other team members.
  • Participate in daily huddles, reviewing high-risk groups, pending referrals, laboratory or testing needs, immunization needs, and visit reasons.
  • Participate in monthly care-team meetings and provide patient updates as needed.
  • Check examination rooms and verify that computers are operational before patient visits; report issues to the nurse supervisor or designee.
  • Escort patients to and from examination rooms and prepare them for clinician visits, verifying names, dates of birth, and demographics.
  • Verify and update the assigned primary care provider in the electronic health record.
  • Obtain and document vital signs, height, weight, and body mass index accurately in the patient medical record.
  • Document pertinent patient information in electronic health records, including visit reasons, medical history, medication updates, allergies, screenings, and Patient-Centered Medical Home information.
  • Complete the Patient-Centered Medical Home tab in the electronic health record, provide education sheets to high-risk patients, and reinforce provider-set goals.
  • Obtain an updated medication list at every visit, notify the provider of changes, and complete medication reconciliation documentation when there are no changes.
  • Collect patient forms and documents requiring provider review and signature and inform the provider.
  • Prepare medical records for clinicians, including laboratory and diagnostic reports and stamped progress notes.
  • Obtain pending imaging or testing completed since the patient's last visit from the patient or caregiver.
  • Perform phlebotomy for adult and pediatric patients and collect other clinician-ordered specimens in compliance with Department of Health and Clinical Laboratory Improvement Amendments protocols.
  • Perform electrocardiograms and other office procedures according to clinician orders.
  • Administer vaccinations, medication standing orders, purified protein derivative plantings, and other tasks within the scope of practice as ordered by the provider.
  • Complete patient intake packets and updates, supporting documentation, and corresponding electronic health record entries.
  • Assist with patient examinations and therapeutic interventions as directed by the clinician or nurse, using trained techniques to ensure patient and staff safety.
  • Assist with check-out by printing patient plans, referrals, and prescriptions and directing patients to the front desk for return visits and internal referrals.
  • Enter prescription and form requests into the designated database and complete associated documentation.
  • Obtain prior authorization for non-covered medications or feeding supplies as needed.
  • Notify patients of laboratory results as directed by the provider and document communications in the electronic health record.
  • Maintain cleanliness and organization of treatment rooms and practice areas, and handle sharps-disposal units and biohazardous waste for removal.
  • Promote continuity of care and act as a liaison among the clinic, patients, families, caregivers, and involved agencies.
  • Participate in telephone triage and ensure requests are appropriately made, prioritized, and supported with required documentation.
  • Document hospital and emergency-room follow-up in the designated electronic health record template and obtain discharge summaries before visits.
  • Review hospital discharge information with providers, assess whether the visit was avoidable, and document the outcome in the patient's chart.
  • Participate in quality-improvement activities as directed.
  • Resupply and inventory materials as assigned.
  • Comply with Occupational Safety and Health Administration, Clinical Laboratory Improvement Amendments, and New York State Department of Health regulations and protocols.
  • Report safety and health hazards promptly to appropriate personnel.
  • Assist with emergencies as directed by the charge nurse, designee, or clinician.
  • Check refrigerator and freezer temperatures daily.
  • Check and sign off daily emergency equipment and check the emergency cart monthly; ensure the crash cart and emergency medications are current.
  • Check vaccine inventory and expiration dates weekly or as directed, ensure vaccines are labeled correctly, and keep vaccine information statements updated.
  • Maintain clinical competencies through on-site training, continuing education, and review of reference materials.
  • Endorse and receive information before and after lunch or breaks.
  • Review and respond to emails on the day they are received.
  • Complete assigned tasks daily and perform other duties assigned by the supervisor or designee.

  • Experience working with individuals with intellectual and/or developmental disabilities.
