Requirements
  • Some credits from an accredited college or university, or at least two years of experience working with homeless individuals.
  • Ability to work with clients from diverse cultural, ethnic, and socioeconomic backgrounds.
  • Ability to work with homeless clients in the field.
  • Ability to communicate effectively, both verbally and in writing, with collaborative entities.
  • Working knowledge of Microsoft Word, Excel, and the Internet.
  • Experience working with homeless persons with co-occurring mental health, physical health, developmental disability, and substance-related issues.
  • Ability to report legal issues in accordance with applicable law, including Tarasoff situations, child abuse, vulnerable adult abuse, danger to self or others, and grave disability.
  • Professional experience working within a multidisciplinary team.
  • Ability to coordinate submission of client documentation if unable to work.
  • If in recovery, at least three years of being drug- and alcohol-free is required.
  • Ability to produce verification of employment eligibility and pass a background clearance.
  • TB test required within the stated pre- or post-hire timeframe and annually thereafter.
  • CPR and First Aid certification required within 30 days of employment.
  • Valid driver's license and auto insurance required.
  • Knowledge of evidence-based practices and ability to use them in the field.
  • Ability to work evenings, overtime, weekends, or holidays as needed.
  • Ability to work in homeless encampments and other challenging field environments, including exposure to severe disabilities, infectious diseases, aggressive or hostile clients, high-crime areas, weather, noise, vehicles, odors, fumes, and pollen.
  • Ability to carry up to 25 pounds and perform sitting, standing, typing, writing, walking, driving, listening, and speaking activities.
  • Ability to perform effectively amid unpredictable situations, crises, distractions, interruptions, and changing priorities or schedules, and to process information, think, and conceptualize effectively.
Responsibilities
  • Conduct immediate-needs assessments with adult consumers and complete comprehensive treatment plans.
  • Present cases within a multidisciplinary team and collaborate with internal and external entities.
  • Provide assistance to homeless clients in the field.
  • Establish client treatment plans that correlate with clinical assessments from therapists, nurses, and substance use counselors.
  • Assist consumers in resolving or coping with mental, emotional, medical, and substance-related problems.
  • Document according to E6 program standards, including entering notes, treatment plans, and outcomes into the Homeless Management Information System.
  • Provide case management services, including immediate-needs solutions, benefits linkages, housing supports, mental and physical health and substance-use program linkages, food and hygiene support, and employment or school-resource linkages.
  • Participate in required meetings, trainings, collaborations, and multidisciplinary team meetings; discuss, educate, present, or evaluate the nature and progress of presenting issues.
  • Coordinate program activities to meet contractual agreements, laws, regulations, and funding requirements with the Program Manager.
  • Participate in regular supervision with the Program Manager or Program Supervisor.
  • Conduct all interventions and treatments in the field.
  • Coordinate transportation and transport E6 staff and participants using the agency van as needed.
  • Maintain documentation of homeless status for intake workers and clients and keep copies in client files.
  • Provide HIV/AIDS information, education, and linkages to testing, housing, and services.
  • Maintain knowledge of HIV/AIDS services and testing locations.
  • Enter outreach activity into the Homeless Management Information System and ensure accurate entry by team members.
  • Assist with collecting and entering information for Los Angeles Homeless Services Authority reports, including Quarterly Performance Reports and Annual Performance Reports.
  • Build rapport with staff and individuals from community organizations serving homeless individuals, families, low-income households, and other HOPICS target populations.
  • Inform, educate, and refer homeless individuals and families to HOPICS programs and available services.
  • Maintain professional boundaries, follow the organization's code of ethics and core values, and represent the agency at meetings and community events.
  • Maintain client files and service records in compliance with HIPAA, 42 CFR Part 2, and funding requirements.
  • Perform other duties assigned by the immediate supervisor or Division Director and support administrative and data-related Information Technology Department functions.
