Requirements
  • A high school diploma or equivalent is required.
  • Case Manager certification is required; uncertified hires must obtain certification within the first 120 days.
  • CPR certification is required post-hire.
  • Valley de-escalation certification is required post-hire.
  • Driving positions require a minimum age of 21 and a current driver's license.
Responsibilities
  • Provide psychosocial rehabilitative services as described by the Medicaid manual.
  • Support clients in following individualized care plans and identifying case management goals.
  • Complete documentation on services provided within Valley's standards.
  • Meet productivity expectations as assigned by the supervisor.
  • Distribute medications as necessary.
  • Transport clients to appointments as necessary.
