Requirements
  • At least one year of experience as a registered nurse in acute care hospital, oncology, long-term care, hospice, or home health; school internships and clinical hours may be considered in individual cases.
  • Current license to practice nursing in the state of requested employment.
  • Current CPR certification.
  • Current automobile insurance and a valid driver’s license.
  • Current transportation maintained for work.
  • Knowledge of terminally ill patients, their families, hospice care, hospice disciplines, community resources, general nursing practice, managed care principles, and regulatory guidelines including Medicare, Medicaid, and ACHC.
  • Ability to perform patient assessments, apply hospice-specific knowledge, communicate effectively, document clinical information, operate nursing assessment tools and office equipment, and work in patient homes or institutional settings.
  • Ability to support patients and families facing terminal illness and crisis, adapt to changing tasks, and work cooperatively with the hospice team.
Responsibilities
  • Serve as the primary point of contact for hospice patients and families throughout their hospice journey.
  • Deliver and document skilled hands-on nursing care according to individualized plans of care in patients’ homes, assisted living settings, or other care locations.
  • Perform comprehensive initial, ongoing, in-person, and telephonic patient assessments and reassessments.
  • Deliver skilled nursing interventions and symptom management aligned with goals of care.
  • Develop and update individualized plans of care, including visit frequency and risk identification.
  • Coordinate care with physicians, nurse practitioners, licensed practical nurses, certified nursing assistants, social workers, chaplains, and other interdisciplinary team members.
  • Provide emotional, physical, and spiritual support, education, comfort, and bereavement guidance to patients and their families.
  • Teach patients, caregivers, and families about disease processes, self-care, end-of-life care, medications, nutrition, dietary needs, symptom management, hospice services, and end-of-life expectations.
  • Communicate changes in condition and signs or symptoms of decline to appropriate providers and the care team.
  • Support continuity of care, hospice and palliative referrals, home health coordination, and transitions between levels of care.
  • Maintain timely and accurate clinical records, including screening and assessment findings, physician orders, progress notes, and care plans.
  • Follow infection-control, regulatory, compliance, and organizational policy standards.
  • Participate in quality-improvement, scheduling, interdisciplinary-group, on-call, and patient-support activities.
  • Maintain effective working relationships with the interdisciplinary group and involve the team when the patient’s care plan requires broader participation.
