Requirements

Responsibilities
  • Build trusted relationships with older adults and their families through listening and empathy.
  • Serve as a resource for navigating medical systems, social services, and emotional wellness needs.
  • Create individualized care plans addressing clinical needs and social determinants of health, including access to food and housing programs, transportation coordination, medication management and reminders, appointment scheduling and follow-ups, and caregiver support and education.
  • Collaborate with therapists, care coordinators, and the technology platform to deliver seamless, person-centered care.
  • Provide feedback to help shape the patient advocacy model as the organization scales.
