Requirements
  • Two years of experience within the last three years as a registered nurse case manager.
  • Experience working in a complex organizational environment and understanding labor relations, military operations, the Military Health System, TRICARE, and the Department of Veterans Affairs as they pertain to the Reserve Component.
  • Ability to work varied shifts per rotation to provide on-site workplace services.
  • An Associate Degree of Nursing is required.
  • Graduation from a college or university accredited by the Accreditation Commission for Education in Nursing or the Commission on Collegiate Nursing Education.
  • For services involving clients in other states, such as telephonic case management, licensure or certification in the state where the client or patient is physically located is required.
  • Possess one of the listed professional certifications: Certified Case Manager, Certified Disability Management Specialist, Certified Rehabilitation Registered Nurse, Certified Occupational Health Nurse, Certified Occupational Health Nurse-Specialist, Advanced Certification in Continuity of Care, Certified Rehabilitation Counselor, Nurse Case Manager, or Care Manager Certified.
  • Ability to travel.
Responsibilities
  • Serve as the administrative point of contact for psychological health-related programs, facilitate and track activities, report results, and liaise with behavioral health programs, boards, and directives.
  • Coordinate and consult with Reserve Component leadership on behavioral health case management and psychological health initiatives and policy.
  • Plan and coordinate regional case management-related meetings and provide verbal and written reports of significant matters.
  • Maintain knowledge of state laws governing mental health issues and clinical practice for the served population, using available clinical supervision and consultation to integrate those laws into service delivery.
  • Carry out Clinical Case Management responsibilities and support training and other Reserve Component-sponsored events such as Yellow Ribbon.
  • Provide training, engage in health promotion activities, assess, consult, and refer clients as needed; collaborate with government support programs and personnel; and provide consultation to commanders, supervisors, and others across Reserve Component levels.
  • Assist Soldiers in establishing treatment plans to support optimal mental fitness and readiness and provide condition and disease-management education, including medication education.
  • Facilitate access to care and services, advocate for Soldiers, and monitor treatment plans for effectiveness.
  • Coordinate with Soldiers, providers, and commanders to maintain engagement in definitive treatment, obtain provider feedback, and inform commands about Soldiers’ readiness status.
  • Evaluate continued service needs, Soldier progress, and readiness status, and consult with the Soldier’s command regarding retention or separation criteria using Command Directed Behavioral Health Evaluations when appropriate.
  • Participate in critical incident response when qualified and ensure that TEM and CISI services are provided as appropriate.
