- The physician must hold a current valid California physician and surgeon license issued by the Medical Board of California or Osteopathic Medical Board of California.
- The physician must have at least two years of full-time supervisory or managerial experience in hospice, a home health agency, or palliative care within the last five years.
- The physician must have had no disciplinary action within the last seven years and must not have concurrent employment with another hospice.
- The physician must hold a doctorate degree in allopathic or osteopathic medicine from an accredited medical school.
- The physician must hold board certification in a specialty recognized by the American Board of Medical Specialties or the American Osteopathic Association Bureau of Osteopathic Specialists.
- The physician must have a current and unrestricted medical license in the practicing state.
- The physician must have a valid National Provider Identifier and Drug Enforcement Administration registration for controlled substances.
- The physician must have current Medicare Provider Enrollment and Medicaid certification if required.
- The physician must have current cardiopulmonary resuscitation certification and TB clearance.
- The physician must demonstrate extensive knowledge of hospice and palliative care principles, regulations, and best practices.
- The physician must have strong organizational skills to manage multiple priorities and regulatory requirements.
- The physician must demonstrate proficiency in healthcare compliance, quality improvement, and ethical considerations.
- The physician must be able to collaborate with attending physicians, staff, and community healthcare providers.
- The physician must meet applicable state-specific requirements and safety-sensitive position requirements.
- Implement and oversee patient Plans of Care to meet medical needs.
- Assess and regularly reevaluate patients’ medical needs.
- Supervise and communicate with contracted and employed hospice physicians and other hospice providers.
- Assist in establishing and reviewing patient Plans of Care.
- Serve on the Interdisciplinary Group and contribute to patient-focused care planning.
- Attend and conduct clinical training sessions.
- Assess hospice eligibility and complete terminal illness certifications and recertifications when needed.
- Act as a medical resource and liaison for the Interdisciplinary Group, attending physicians, and community medical providers.
- Facilitate meetings with patients’ family members and provide guidance for quality care.
- Provide direction and oversight for patient care and ensure that Plans of Care are implemented and medically appropriate.
- Hospice and Palliative Medicine board certification and/or Hospice Medical Director Certification Board certification is preferred.
- Primary care board certification is preferred.
