Requirements
  • Current and active certification as a Certified Nursing Assistant in the State of New York.
  • Effective written and oral communication skills, including the ability to appropriately document services provided for billing and tracking purposes.
  • Ability to meet the essential physical demands of the position, including frequent walking, sitting, standing, bending, twisting, stooping, kneeling, crouching, pushing, pulling, reaching, handling objects and controls, assisting with lifting or moving individuals of any weight with mechanical lifts or other equipment, pushing individuals in wheelchairs, lifting or moving up to 25 pounds occasionally, and implementing emergency interventions under a person's Behavior Support Plan.
  • Ability and willingness to assist with a variety of medical interventions and supports, including regular exposure to and handling of bodily fluids.
  • Must hold or obtain and maintain Strategies for Crisis Intervention and Prevention revised, Approved Medication Administration Personnel, Medical Immobilization Protective Stabilization, cardiopulmonary resuscitation, and First Aid certifications within one year of appointment.
  • Must hold or obtain and maintain other certifications required by the program assignment and supported individuals, such as Diabetes Care, Tube Feeding, Catheterization, and Colostomy certifications.
Responsibilities
  • Ensure the health, wellness, and safety of supported individuals by providing appropriate supervision and managing crisis situations in accordance with policies, procedures, and regulations.
  • Maintain a clean, safe, organized, and therapeutic environment.
  • Implement person-centered individualized plans and assist with personal care, activities of daily living, intimate care, personal hygiene, laundry, and housekeeping.
  • Ensure medications are administered as prescribed and in accordance with applicable guidelines.
  • Teach and develop skills, support person-centered advocacy, assist with communication and dispute resolution, and facilitate ethical decision-making.
  • Supervise or plan and prepare balanced meals and snacks in accordance with individual dietary restrictions and guidelines, including modified diets and liquid consistencies.
  • Transport and escort individuals to recreational activities, medical appointments, and home visits.
  • Assist individuals with purchases and safeguard personal allowance funds and other provided funds.
  • Under the supervision of a Registered Nurse or Licensed Practical Nurse and with appropriate training, provide additional supports for individuals with complex medical needs according to applicable plans of nursing service and behavior support plans.
  • Assist with transfers, turns, and lifts involving beds, wheelchairs, prescribed equipment such as Hoyer lifts, and agency vehicles using safe person-handling procedures.
  • Gather and stock medical supplies.
  • Measure and document food and liquid intake and urinary and fecal output, and report changes to medical providers or nursing staff.
  • Check vital signs, including temperature, blood pressure, and heart rate.
  • Schedule and follow up on appointments in coordination with the Health Care Specialist and appropriate nursing staff.
  • Perform catheter care.
  • Assist with ostomy care, including colostomies and ileostomies.
  • Provide enteral nutrition, including tube feedings.
  • Provide wound care according to medical provider orders.
  • Clean rooms and bed linens and handle and dispose of sharps, bodily waste, and bodily fluids in accordance with Occupational Safety and Health Administration guidelines and the Exposure Control Plan.
  • Collect, record, and summarize data on individuals' observable behavior.
  • Observe and report changes in behavior and other signs and symptoms to the clinical or leadership team.
  • Complete and maintain required training and certifications, including specialized program and position training, and apply the acquired skills.
  • Review communication logs daily and record events, progress notes, services, and other information; collect and enter operational data using an electronic health record.
  • Complete required paperwork and documentation accurately and on time for compliance and billing, including notifications, logs, incident reports, file updates, maintenance requests, phone calls, and incident tracking.
  • Use foundational teaching skills such as EMOE, task analysis, essential skills, supports, modifications, and progressive teaching.
  • Perform additional duties of a Direct Support Professional 1 and meet and demonstrate OPWDD Core Competencies.
  • Comply with Federal, State, Local, and other applicable regulatory requirements, including the Health Insurance Portability and Accountability Act.
  • Perform other duties as assigned.
