Requirements
  • A bachelor's degree in a health-related field of counseling, psychology, social work, or another behavioral health field is required.
  • A bachelor's degree in criminal justice is accepted with fifteen college-level semester hours of behavioral-health coursework.
  • A valid Tennessee Driver's License is required.
  • An acceptable Motor Vehicle Report is required.
  • An acceptable criminal background investigation is required.
  • Personal automobile insurance is required.
  • A cell phone is required.
  • Transportation seating four people is required.
Responsibilities
  • Continually assess consumers’ strengths, needs, supports, and goals, including meeting with consumers and their supports in the community.
  • Provide support to consumers in their homes, at school, at court, at appointments, in the office, and at other community-based locations where support is needed.
  • Collaboratively complete comprehensive and individualized care plans, crisis plans, the PRAPARE screening tool, the Columbia Suicide Severity Risk Scale, and DLA/CANS assessment tools within 30 days of admission and every six months thereafter, covering physical health, behavioral health, and other applicable domains.
  • Assist with referrals and linkage to internal and community resources to ensure consumers are connected to and receiving needed supports.
  • Provide side-by-side support to consumers in addressing barriers and learning new skills.
  • Advocate for consumers when necessary.
  • Assist consumers with attending clinic, therapy, IHC/PCP appointments by providing reminders, addressing barriers to attendance, and transporting them when needed.
  • Educate consumers about gaps in care and collaboratively develop action plans to close gaps and manage health proactively.
  • Educate consumers and their supports about appropriate use of emergency departments, inpatient services, primary care providers, and urgent care.
  • Provide supportive interventions when consumers use emergency-department or inpatient services, according to agency guidelines.
  • Ensure consumers’ daily psychiatric, medical, and environmental needs are planned for and coordinated.
  • Ensure timely and coordinated transitions for consumers within the agency and community.
  • Meet productivity standards specified in agency policies and procedures.
  • Meet monthly with the Care Management Supervisor for supervision and scheduled meetings, prepared with questions, caseload information, and solutions to challenges.
  • Attend scheduled daily in-person team meetings prepared with questions, caseload information, and solutions to challenges.
  • Participate in collaborative problem-solving and team-building activities.
  • Collaborate with team members, agency employees, and community partners to promote consumer success in accessing care and progressing toward goals.
  • Follow up on plans developed in supervision and team meetings within assigned deadlines.
  • Respond to urgent and crisis consumer needs with guidance from the supervisor, crisis services, and clinic staff by developing and implementing plans and interventions.
  • Develop and maintain positive, professional relationships with consumers’ supports.
  • Complete timely and appropriate agency documentation.
  • Develop and maintain community relations through prompt follow-through with contacts.
