Requirements
  • At least one year of registered nurse experience in acute care hospital, oncology, long-term care, hospice, or home health; school internships and clinical hours may be considered in individual cases.
  • A current license to practice nursing in the state of requested employment is required.
  • Current CPR certification is required.
  • Current automobile insurance and a valid driver’s license are required.
  • Knowledge of terminally ill patients and their families, hospice principles, the roles of hospice disciplines, community resources, general nursing practice, managed care principles, and regulatory guidelines including Medicare, Medicaid, ACHC, and human resource management principles is required.
  • Excellent patient assessment skills, documentation and program management skills, and effective oral and written communication skills are required.
  • The nurse must be able to operate nursing assessment tools, computers, facsimile equipment, copiers, and a cell phone or beeper.
  • The nurse must maintain transportation for work.
  • The position requires flexibility in performing changing tasks, effective communication with people from all socioeconomic backgrounds, and the ability to work safely in patient homes or institutional settings with potential exposure to infections, communicable diseases, odors, blood, excreta, and hazardous materials.
Responsibilities
  • Serve as the primary point of contact for patients and families throughout the hospice journey.
  • Deliver and document skilled hands-on nursing care according to each patient’s individualized plan of care in homes, assisted living facilities, or other patient settings.
  • Perform comprehensive initial, ongoing, in-person, and telephone registered nurse assessments and reassessments, including screenings.
  • Deliver skilled nursing interventions and symptom management aligned with goals of care.
  • Develop and update individualized plans of care, including visit frequency and risk identification.
  • Collaborate with physicians, licensed practical nurses, certified nursing assistants, social workers, chaplains, and other interdisciplinary team members to build and refine care plans.
  • Provide emotional, physical, and spiritual support, education, comfort, and bereavement guidance to patients and loved ones.
  • Teach patients, family members, caregivers, and loved ones about disease processes, self-care techniques, end-of-life care, medications, nutrition, dietary needs, and safe caregiving.
  • Communicate changes in patient condition and signs or symptoms of decline to the appropriate provider, such as a nurse practitioner, primary care provider, or hospice physician.
  • Support continuity of care by recognizing and supporting home health, palliative, and hospice referrals and transitions between levels of care.
  • Document timely and accurate clinical records, including screening and assessment findings, physician orders, progress notes, and care plans.
  • Follow infection-control, regulatory, policy, and compliance standards.
  • Maintain effective working relationships with the interdisciplinary group and involve the team when the patient’s care plan warrants team participation.
  • Participate in quality-improvement, scheduling, on-call, and patient-support activities, conducting on-call services in a clinically competent and responsive manner.
  • Help patients and families understand medication administration, hospice philosophy and services, symptom and pain management, and end-of-life processes and expectations.
