  • Bachelor of Science in Nursing.
  • Must reside in a compact Nurse Licensure Compact state.
  • Active Compact Registered Nurse license in the state of residence in good standing with the nursing board of that state.
  • At least 5 years of nursing experience.
  • At least 2 years of experience working in care, case, and disease management, preferably in a health plan, health navigator, or third-party administrator environment.
  • At least 2 years of experience working in labor and delivery.
  • Comfort discussing a wide variety of medical conditions.
  • Ability to work remotely and comfortably use technology.
  • Ability to practice empathy and reassure patients and their families during challenging times.
  • Ability to work efficiently, meet volume goals, and balance priorities without sacrificing quality.
  • Ability to work flexibly in a rapidly changing environment.
  • Strictly follow security and HIPAA regulations to protect patients’ medical information.
  • Strong verbal and written communication skills, including the ability to gather a clinical history, answer patient questions at a patient level, and succinctly summarize findings.
  • Strong competence using multiple computer and medical record systems.
  • Ability to collaborate across multidisciplinary clinical and non-clinical teams.
  • Ability to understand cultural and socioeconomic issues affecting members and coordinate available resources.
  • Excellent grammar, attention to detail, and ability to write medical information in easy-to-understand, patient-centered language.
  • Prompt and regular attendance at the assigned work location.
  • Capability to remain seated in a stationary position for prolonged periods.
  • Eye-hand coordination and manual dexterity to operate a keyboard, computer, and other office-related equipment.
  • Capability to occasionally exert approximately 20 pounds of force, such as lifting a computer or laptop.
  • Capability to work appropriately with leadership, employees, and members.
Responsibilities
  • Deliver coordinated, patient-centered virtual care management by telephone and/or messaging to improve members’ health outcomes.
  • Generate care plans with members and the multidisciplinary care team and help members achieve their desired goals.
  • Help members navigate complex medical conditions, treatment pathways, benefits, and the healthcare system in general.
  • Partner with members’ local providers to ensure coordinated care.
  • Provide compassionate, longitudinal follow-up care and build supportive relationships.
Desired Qualifications
  • Additional active licensure, including in Illinois, California, Minnesota, or New York.
  • Certified Case Manager certification.
  • Experience working remotely.
  • Comfort with technology.
