Requirements
  • Certification as a Medication Aide or compliance with state requirements for a medication aide.
  • A minimum of one year of experience working with elders or disabled individuals is preferred.
  • Demonstrated leadership capabilities.
  • Knowledge and experience in the assisted living industry and dementia care or skilled nursing is preferred.
  • Ability to work weekends, evenings, and flexible hours during peak service delivery days and times.
Responsibilities
  • Identify ongoing resident needs and services through the assessment and Personalized Service Plan process.
  • Ensure proper follow-through and documentation for residents whose clinical care needs change.
  • Support the Wellness Nurse by completing the clinical sections of monthly wellness visits and Personalized Service Plans accurately.
  • Inform the Wellness Director and/or Executive Director of medication or service-level changes that may result in a higher tier level or potential move-out.
  • Assist with obtaining monthly weights and vital signs for each resident before completion of the monthly wellness visit.
  • Assist with resident care as needed.
