Requirements
  • A Master's Degree in Social Work is required.
  • At least five years of clinical social work experience with the geriatric population is required.
  • An unrestricted Licensed Clinical Social Worker license in the applicable state or states is required.
  • Age-specific competency in working with elderly people and knowledge of community services for elderly people and their families are required.
  • The ability to conduct psychosocial assessments and develop and implement viable care plans is required.
  • Familiarity with electronic health record medical documentation is required.
  • Strong computer skills, including Microsoft Word, Excel, and PowerPoint, are required.
  • Strong verbal, written, presentation, and interpersonal communication skills are required.
  • The ability to work effectively in a team environment is required.
  • Knowledge of social work principles and practices, including case management and counseling techniques, is required.
  • The ability to empathize with clients is required.
  • The ability to provide after-hours services as needed is required.
  • The ability to leverage internal and external resources as part of a patient's treatment plan is required.
  • Experience writing assessments and reports to monitor client progress is required.
  • A valid driver's license, access to a car, willingness to drive to patient homes or other patient locations, and active auto insurance are required.
  • The ability to travel locally to patient locations, potentially up to 80% of the time, is required.
  • The ability to lift up to 20 pounds is required; moving, lifting, or transferring patients may involve lifting up to 50 pounds and assisting with weights over 100 pounds.
  • The ability to stand for extended periods, drive to patient locations, and meet fine-motor and visual-acuity requirements is required.
  • Candidates must be legally authorized to work in the United States without employer sponsorship now or in the future.
  • Successful completion of pre-employment drug screening and applicable ongoing workplace drug testing is required.
  • Successful completion of a background check and continued satisfactory FACIS checks are required.
Responsibilities
  • Practice the WellBe mission by helping patients lead healthier, meaningful lives through complete care.
  • Perform psychosocial assessments and provide therapeutic services to patients and their families.
  • Work closely with the Behavioral Health Medical Director to manage complex patient cases and transitions of care.
  • Serve as a resource for WellBe communities managing complex behavioral-health cases.
  • Manage complex behavioral and psychosocial needs to improve clinical and financial outcomes.
  • Maintain communication with patients while addressing their concerns and goals and helping them maintain positive attitudes.
  • Build rapport with patients and their families and relay concerns to the WellBe provider and interdisciplinary team.
  • Assist members in effectively using available resources to meet personal health goals and develop their capabilities.
  • Monitor patient progress and adjust treatment plans as needed.
  • Coordinate with healthcare providers, agencies, and community resources to create treatment plans addressing social, cultural, and financial needs.
  • Collaborate with the interdisciplinary team and participate in regularly scheduled team meetings.
  • Ensure compliance with regulatory agencies and WellBe clinical guidelines.
  • Partner with the community team to refer patients to appropriate no-cost or low-cost community services, including Meals on Wheels, Area Agencies on Aging, senior assistance programs, and transportation resources.
  • Involve patients' families and primary caregivers as needed to achieve appropriate care decisions and outcomes.
  • Collaborate with people involved in members' care, including home health, hospice, and community-agency contractors, to meet patients' care goals.
  • Consult with and advise the clinical team regarding the relationship of social, emotional, and cultural factors to health and medical care and the availability of community social services.
  • Comply with safety policies and procedures and report potential or actual unsafe acts or conditions to a supervisor or team.
  • Meet or exceed WellBe productivity, customer-service, quality-assessment, and performance targets.
  • Participate in community activities that link WellBe to its communities.
  • Maintain current written progress notes and other documentation in members' medical records, including signed and dated documentation on the day services are provided.
  • Participate in program and policy development for the WellBe Social Work program.
  • Perform other tasks needed to accomplish the team's objectives and goals.
Desired Qualifications
  • Bilingual ability in English and Spanish is preferred.
  • Two years of social work experience, preferably in health care, and at least one year working with a frail or elderly population is preferred.

The company now serves over 120,000 patients in eight states. The new hires include Shafina Shehnaz as chief financial officer, bringing experience from Optum Health where she served as CFO of the care delivery arm.
