About the job

Requirements
  • 10+ years of professional software engineering experience, including several years designing and operating large-scale, production distributed systems.
  • A track record of owning architecture for a product or platform end to end, including making foundational technical decisions and evolving them over time.
  • Deep expertise in backend systems, including service design, RESTful and asynchronous APIs, event-driven architectures, task queues, and data modelling across relational and non-relational stores.
  • Strong grasp of reliability engineering, including observability, fault tolerance, capacity planning, and incident-informed design for systems with strict correctness and latency requirements.
  • Hands-on proficiency in at least one core language: Python or Go; comfort reading and reviewing code across the stack, including React-based frontends.
  • Working knowledge of cloud-native infrastructure such as GCP, AWS, or Azure, Kubernetes, and infrastructure-as-code, including their operational implications.
  • Experience building or integrating AI/LLM-backed systems, or clear evidence of quickly developing depth in agent orchestration, evaluation, guardrails, and inference trade-offs.
  • Strong security instincts, including designing for data isolation, encryption, access control, and auditability.
  • Ability to communicate in writing and verbally, carry designs from whiteboard to ADR to aligned teams, and explain trade-offs to founders and customers.
  • Ability to tackle complex, ambiguous technical problems and drive them to clear decisions.
Responsibilities
  • Own the end-to-end architecture of the AI agents platform, including services, APIs, data models, asynchronous task pipelines, LLM and voice agent orchestration, and integrations with payer systems, EMRs, and telephony.
  • Design for reliability, latency, and safety by defining patterns for fault tolerance, idempotency, retries, observability, and graceful degradation across production agent workflows.
  • Set the architecture for multi-tenancy, data isolation, and Protected Health Information handling in partnership with the Security and Compliance Lead, incorporating HIPAA and SOC 2 requirements.
  • Make and document technical decisions about build versus buy, service boundaries, storage and queueing choices, LLM providers, and inference strategy through design reviews and architecture decision records.
  • Prototype critical paths, write production code on ambiguous and high-risk problems, and unblock teams facing difficult technical issues.
  • Partner with the platform team on Kubernetes infrastructure, including autoscaling, deployment topology, cost, and service-level objectives for agent and voice workloads.
  • Mentor senior engineers, drive code and design review culture, and define standards for testing, evaluation, and release safety across teams.
  • Translate product and customer needs into a technical roadmap and sequence architectural investments against business priorities.
  • Anticipate scaling challenges involving call volume, customers, and team size, and establish the required technical foundations in advance.
Desired Qualifications
  • Experience with voice AI, telephony such as SIP or WebRTC, or real-time systems.
  • Prior work in U.S. healthcare or other regulated domains, including HIPAA, SOC 2, or HITRUST, and familiarity with EMRs, payer integrations, FHIR, or HL7.
  • Experience with LLM evaluation frameworks, prompt or fine-tuning pipelines, and hallucination and safety controls in production.
  • Experience scaling engineering teams and architecture together through platformization, service extraction, and developer experience improvements.
  • Open-source contributions, technical writing, or conference talks.

About the company

What believers are saying

  • Mayo Clinic Platform_Accelerate selected 100ms in February 2026 for a 30-week cohort.
  • Release notes show active SDK shipping through August 2026 across Web, React Native, Android.
  • Healthcare AI expands 100ms beyond crowded conferencing APIs into higher-value workflows.

What critics are saying

  • 100ms pivoted from live-video infrastructure to healthcare AI, risking customer confusion and churn.
  • No public layoffs or new funding since March 2022 limits runway visibility.
  • If healthcare sales fail, 100ms becomes a stranded rebrand with legacy SDK inertia.

What makes 100ms unique

  • Mayo Clinic platform access gives 100ms clinical validation unavailable to generic video vendors.
  • Founders built video infrastructure at Disney+ Hotstar and Facebook, proving scale expertise.
  • HIPAA-compliant US data residency and human escalation fit regulated specialty-practice workflows.
