Who You Are 

You've spent your career at the intersection of purpose and practice — in community health outreach, nonprofit or public-health consulting, or social-impact strategy — helping organizations turn intention into measurable change.  

This is not an account management role. You are the primary relationship holder for a portfolio of 20–25 CareStation partners. From onboarding forward, you and the partner set shared goals for the impact the CareStation should create — appropriate to each partner's objectives and circumstances — and you hold both OnMed and the partner accountable for reaching them. You bring what you learn in the field back to shape how OnMed builds and deploys. 

The Role 

As Partner Impact Manager, you own the day-to-day relationship for a portfolio of CareStation partners — setting shared goals at onboarding, tracking outcomes, and bringing field insight back into how OnMed builds and deploys. 

Reporting Structure

This role reports to our Partner Impact Director. 

Key Responsibilities 

• Serve as the primary relationship holder for a portfolio of 20–25 CareStation partners 

• Guide partners through onboarding, embedding the CareStation into their broader community health strategy — not just installing it 

• At the start of each partnership, establish a clear, shared objective and mutually agreed goals appropriate to the partner's objectives and specifics — so success is defined up front 

• Ensure clear mutual understanding of roles and responsibilities during the duration of the program and hold OnMed and the partner accountable 

• Track and analyze partner-level outcomes data to spot what's working, what's stalling, and where to focus next 

• Run forward-looking, mission-grounded partner reviews — not backward-looking report-outs 

• Bring partner insight back into OnMed, shaping how the roadmap evolves to meet partner and community needs 

• Identify moments where a partner is ready to expand their commitment and help them build the case internally 

• Contribute to OnMed's broader impact narrative by documenting community outcomes across your portfolio 
• Perform other related role's responsibilities as assigned 

Knowledge, Skills and Abilities 

• Demonstrated ability to influence without authority — you move partners toward shared goals through trust and clarity, not hierarchy 

• Experience helping organizations translate mission into measurable outcomes 

• Comfort operating in complex, multi-stakeholder environments — health systems, government agencies, community organizations 

• Strong written and verbal communication — able to distill complexity for a C-suite audience and a community health worker in the same day 

• Genuine personal commitment to health equity and community access 

• Direct experience in community health, FQHCs, public health programs, or social determinants of health initiatives a plus 

• The confidence to hold a partnership — yourself and your partner alike — to a high bar, while deepening the relationship 

• Comfortable working in data and analytics tools to track partner health, surface trends, and back up recommendations with numbers, not just instinct 
• Must be a native-level Spanish speaker, both verbally and in writing to effectively communicate with our Puerto Rico-based partners 

Education and Experience 

• Bachelor's degree required; degree in Public Health, Public Policy, Social Work, or a related field preferred 

• 5+ years in a client-facing or partner-facing role within community health outreach, nonprofit or public-health consulting, or social-impact strategy 

• Background in management consulting, nonprofit strategy, or impact investing with a client-facing component 

• Experience working with underserved communities in a professional capacity 

• You must be fluent in Spanish with professional-level verbal and writing skills 

• Role requires with occasional travel up to 30-40% of time 

OnMed provides a competitive salary and benefits package, including unlimited PTO and paid holidays. 

The base salary for this role is up to $120,000 commensurate with the candidate's experience.
