Requirements
  • A diploma, Associate Degree in Nursing, and/or Bachelor of Science in Nursing from an accredited institution.
  • Three years of nursing experience.
  • Knowledge of universal precautions for infection control and safe nursing standards for self-protection.
  • Ability to manage time effectively and function with a changing daily schedule.
  • Ability to provide patient- and family-focused care and patient/caregiver education.
  • Skill in using an Electronic Medical Record.
  • Current Virginia state Registered Nurse license.
  • A valid driver’s license and reliable transportation with automobile insurance meeting organization requirements.
  • Ability to stand and walk almost constantly for up to 90% of work time and lift and position patients for up to 50% of work time.
  • Ability to perform visits anywhere within the Gilchrist Inova service area.
  • Ability to concentrate, pay close attention to detail, and work in stressful situations.
  • Current licensure and certification must be maintained.
Responsibilities
  • Assess patients in their place of residence or in hospitals and prepare them for entry into Gilchrist Inova’s hospice services.
  • Complete initial assessments of patients and families/caregivers to determine hospice eligibility and needs, including physical, emotional, spiritual, and mental status.
  • Prepare and implement the initial care plan for the patient/family unit with Medical Director approval.
  • Provide direct nursing care to patients to maintain the highest level of care.
  • Inform the Manager about the utilization of other team members, outside agencies, and community resources based on initial assessment findings.
  • Document clinical findings to support initial hospice eligibility and participate in discharge planning when necessary, working with relevant patient care, social services, and other departments.
  • Teach family members, hired caregivers, and patients care skills, including pain control, medication administration and titration within the prescribed range, injections, blood draws for laboratory tests, intravenous management, dressing changes, hygiene, nutrition, and vital-sign monitoring according to physician orders.
  • Arrange for necessary equipment to be available in the home setting.
  • Attend to patients and families at the time of death when it occurs during a visit.
  • Participate in Admissions department projects and process-improvement strategies.
  • Establish and maintain effective working relationships with hospice referral sources in the service area.
  • Meet with healthcare administrators representing Gilchrist Inova in the community.
  • Obtain necessary consents in accordance with applicable regulations and reimbursement policies.
  • Drive to hospitals, skilled nursing and assisted living facilities, and patients’ homes as part of field visits.
  • Maintain effective working relationships with the interdisciplinary care team and support smooth transitions into hospice services.

Advancing Health Equity Through Care, Partnership, and Legal Support About GBMC and Gilchrist. MVLS is proud to recognize GBMC and Gilchrist as its Community Partner of the Month. Together, GBMC Health Partners and Gilchrist expand access to care across Maryland. Their work includes primary care, geriatric medicine, hospice, palliative care, and community-based support. GBMC has served the community for more than 60 years. It is Maryland's only independent community health system. GBMC was founded in 1965. It was created through the merger of Presbyterian Eye, Ear and Throat Charity Hospital and the Hospital for Women of Maryland. In 2019, GBMC Health Partners expanded its Baltimore City presence. The organization opened its Jonestown Primary Care office at the original Presbyterian Eye, Ear and Throat Charity Hospital site. Gilchrist is the serious illness and hospice care organization within the GBMC HealthCare system. It was established in 1994 and opened Gilchrist Center Baltimore in 2021. Together, GBMC and Gilchrist advance health equity. They do this by expanding access to primary care, geriatric medicine, hospice, palliative care, and specialty services. The gift, made through Richard and Linda Offerdahl, supports the Surgery Enhancement Project at Incline Village Community Hospital and helped the foundation reach its $10M fundraising goal; the hospital's new patient registration area will be named for the Offerdahl family. * UH Samaritan Medical Center received a $1M gift from Bob and Jan Archer, establishing the UH Samaritan Medical Center Fund for Hospital Operations. The fund supports healthcare provider recognition, engagement, and career advancement programs; the hospital's main reception area has been named the Robert M. and Janet L. Archer Lobby. * Intermountain Health St. Vincent Regional Hospital received a $5M gift from The Fortin Family Foundation. The leadership gift, announced June 23, will expand access to advanced pediatric services for children and families across Montana, Wyoming, and the western Dakotas as part of the Campaign for St. Vincent Regional Hospital. * GBMC HealthCare received an anonymous $6M gift toward the expansion and renovation of its Pediatric Emergency and Inpatient Departments in Baltimore County.
