About the job
- A diploma, Associate Degree in Nursing, and/or Bachelor of Science in Nursing from an accredited institution.
- A current Maryland state license as a Registered Nurse.
- One to two years of nursing experience.
- Knowledge of universal precautions for infection control and safe nursing practices for self-protection.
- Skill in using an electronic medical record and/or ability to learn.
- Skill in using a Blackberry and portable computer.
- Ability to provide patient- and family-focused care.
- Complete an initial assessment of the patient and family to determine home care needs, including physical, emotional, spiritual, and mental status.
- Prepare and implement the patient/family care plan in cooperation with the Interdisciplinary Group.
- Provide direct nursing care to maintain the highest level of patient care in conjunction with the primary physician.
- Coordinate with the Clinical Team Leader on the use of other team members, outside agencies, and community resources.
- Update the primary physician when necessary and regularly on a monthly basis.
- Update the interdisciplinary team at patient care conferences.
- Teach family members or hired caregivers patient care skills, including pain control; medication management, including titrating medication within the prescribed range; injections; drawing blood for labs; managing IVs; dressing changes; hygiene; nutrition; checking vital signs according to physician orders; and addressing emotional needs.
- Ensure arrangements are made for equipment needed in the home setting.
- Attend to the patient and family at the time of the patient’s death.
- Home Health or Hospice experience is preferred.
