Adult Medical Home Direct Support Professional Role
About the job
- A high school diploma or GED is required.
- A valid and current driver's license and acceptable motor vehicle record are required; the license, insurance, and registration must be aligned with the state of permanent residence.
- A clear criminal record and other background checks in accordance with state requirements are required.
- Assist residents with all activities of daily living.
- Participate in and carry out developmental activities.
- Implement goal plans and behavioral management programs.
- Assist with meal preparation and general housekeeping functions.
- Provide transportation for social and medical appointments.
- Provide overall support to the residents and the program.
- Experience with medically fragile individuals and/or individuals with disabilities is preferred.
