Requirements
  • Be a Registered Nurse with a minimum of one year of RN experience in acute care hospital, oncology, long-term care, hospice, or home health; school internships and clinical hours may be considered in individual cases.
  • Hold a current license to practice nursing in the state of requested employment.
  • Hold current CPR certification.
  • Have current automobile insurance, a valid driver’s license, and transportation available for work.
  • Apply knowledge of terminally ill patients, their families, hospice services, interdisciplinary hospice roles, patient assessment, community resources, general nursing practice, managed care principles, and applicable regulatory guidelines.
  • Operate nursing assessment tools, computers, facsimile equipment, copiers, and cell phones or beepers effectively.
  • Work safely in patients’ homes or institutional settings with potential exposure to infections, communicable diseases, odors, blood, excreta, and hazardous materials.
  • Communicate effectively with patients, families, caregivers, coworkers, and hospice team members across socioeconomic backgrounds.
  • Demonstrate flexibility, tact, cooperation, maturity, and the ability to support patients and families during crises.
  • Meet applicable state-specific requirements and safety-sensitive position requirements.
Responsibilities
  • Serve as the primary point of contact for hospice patients and families throughout their hospice journey.
  • Deliver and document skilled nursing care according to individualized plans of care in patients’ homes, assisted living facilities, or other care settings.
  • Perform comprehensive initial, ongoing, in-person, and telephonic patient assessments and reassessments.
  • Monitor patient condition, identify changes or decline, and adjust care and interventions as needed.
  • Deliver skilled nursing interventions and symptom management aligned with goals of care.
  • Develop and update individualized plans of care, including visit frequency and risk identification.
  • Coordinate care with physicians, nurse practitioners, licensed practical nurses, certified nursing assistants, social workers, chaplains, and other interdisciplinary team members.
  • Provide emotional, physical, and spiritual support, education, comfort, and bereavement guidance to patients and loved ones.
  • Educate patients, family members, and caregivers about disease processes, self-care, end-of-life care, medications, nutrition, dietary needs, hospice philosophy and services, symptom and pain management, and end-of-life expectations.
  • Teach caregivers to safely and confidently care for their loved ones.
  • Communicate changes in condition and signs or symptoms of decline to appropriate providers, including nurse practitioners, primary care physicians, and hospice physicians.
  • Support continuity of care by recognizing and supporting home health, palliative, and hospice referrals and transitions between levels of care.
  • Document timely clinical records, including screening and assessment findings, physician orders, progress notes, and care plans.
  • Maintain effective working relationships with the interdisciplinary group and involve the team when patient care plans require broader participation.
  • Follow infection-control, regulatory, compliance, and policy standards.
  • Participate in quality-improvement, scheduling, on-call, and patient-support activities.
  • Conduct on-call services in a clinically competent and responsive manner.
