Requirements
  • Graduate of an accredited school of nursing.
  • Hold a valid Registered Nurse license.
  • Be qualified as a Qualified Mental Health Professional in Oregon.
  • Have at least one year of related nursing experience.
  • Hold First Aid, cardiopulmonary resuscitation, and infection control certifications.
  • Hold a valid driver's license.
  • Build strong relationships with diverse individuals or communities and understand their unique needs and challenges.
  • Meet the cultural needs of individuals or communities, including language, traditions, and values.
  • Successfully participate in and complete a three-part Nonviolent Crisis Intervention exam and training.
  • Maintain current role-specific and facility-wide trainings and certifications, including nursing licensure, first aid and safety, medication administration, infection control, and cardiopulmonary resuscitation certification.
Responsibilities
  • Collaborate with interdisciplinary teams regarding the medical aspects of client admissions.
  • Communicate medical concerns and coordinate follow-up appointments or referrals with internal and external interdisciplinary teams.
  • Collect and review clients' medical data and ensure medical records are current and accurate.
  • Assess nursing implications of resident data and determine when to communicate with a physician.
  • Monitor acute and chronic medical conditions and provide treatment or consultation as needed.
  • Gather relevant information and assess client status during emergencies, provide appropriate care or treatment, direct others as necessary, and collaborate on referral to a higher level of care when appropriate.
  • Serve as a medical resource to staff and interdisciplinary teams regarding client medical concerns.
  • Collaborate with treatment staff to develop individual client treatment plans.
  • Verify, count, and administer medications to residents.
  • Educate clients about medication side effects, adverse effects, and drug interactions.
  • Monitor clients daily for medication side effects, adverse effects, and drug interactions.
  • Ensure client prescriptions are filled and the appropriate medication quantities are maintained throughout each client's residential stay.
  • Monitor medication logbooks for completion and accuracy.
  • Complete required department-specific, role-specific, and facility-wide training.
  • Participate in internal training opportunities with community partners.
  • Maintain client confidentiality and comply with policies concerning privacy, informed consent, and confidentiality standards for clients and their families.

Nexus Family Healing is a nonprofit provider of mental health and family support for children, young people, and caregivers. Its continuum includes foster care, outpatient and community mental health services, residential treatment, and family stabilization. Programs operate across communities in Minnesota, North Dakota, Illinois, and Maryland. The care model addresses behavioral health, trauma, family stability, and placement needs across residential and community settings, positioning Nexus within nonprofit child and family services rather than as a standalone residential-care operator.
