Gastro of the Rockies partners with Cosan to launch care management programme for chronic digestive conditions

Gastroenterology of the Rockies has partnered with Cosan, an AI-enabled connected care management company, to launch a Principal Care Management programme supporting patients with chronic gastrointestinal conditions between clinic visits. The programme provides ongoing support through an interdisciplinary care team including care managers, nutrition specialists, behavioural health professionals and medication support resources. It targets patients with conditions such as inflammatory bowel disease, liver disease and GERD, focusing on nutritional health, functional health, mental health and medication management. The initiative aims to bridge gaps in care when patients manage conditions independently, enabling earlier identification of issues and timely interventions.
