Requirements
  • All applicants must be eligible for licensure with the College of Physicians and Surgeons of Nova Scotia.
  • Coverage with the Canadian Medical Protective Association or proof of alternative coverage is required.
  • Completion of at least one year of Geriatric Medicine training during Internal Medicine residency or Care of the Elderly certification in addition to Family Medicine is required.
Responsibilities
  • Work collaboratively with the established multidisciplinary Seniors’ Clinic team to provide comprehensive geriatric care to patients aged 65 years and older, with a particular focus on frail older adults.
  • Operate within the bylaws and rules and regulations of Nova Scotia Health Authority.
  • Provide inpatient geriatric and pre-operative geriatric optimization consultations at Colchester East Hants Health Centre.
  • Provide outpatient, office-based geriatric consultations for community-dwelling older adults at Rath Eastlink Community Centre, in person and/or virtually.
  • Conduct case reviews and provide collaborative support with geriatric clinicians.
  • Conduct patient home visits when appropriate.
  • Provide consultative services to long-term care facilities throughout Colchester County.
  • Participate in a shared on-call schedule providing telephone consultation to primary care providers managing older adults in community, long-term care, and acute care settings within the Northern Zone.
  • Provide on-call coverage on weekdays from 8:30 a.m. to 4:30 p.m.
Desired Qualifications
  • CCFP designation from the College of Family Physicians of Canada, or eligibility for certification with the Royal College of Physicians and Surgeons of Canada.
