A Bachelor's Degree in a social behavior science or related field is required.
Three years of applicable experience are required.
A valid Florida Driver's License with six points or less in the last five years is required.
The candidate must pass a Level 2 Background check through the Florida Department of Children and Families and the Agency for Health Care Administration Clearinghouse.
Responsibilities
Assist other team members with Crisis Intervention Services for the client and/or family.
Complete an individualized service plan for each client based on information gathered from the assessment, including long-term desired outcomes and a comprehensive strategy for achieving those outcomes.
Act as an advocate and support person for individuals assigned to the Children's Community Action Team, link them to appropriate services, and monitor progress or lack of progress by the individual and their family.
Serve as an agency liaison with the Florida Department of Children and Families, the Department of Juvenile Justice, and other community agencies to coordinate referrals and organize community presentations.
Assist with transporting youth or family members to medical appointments, court hearings, or other activities outlined in the Plan of Care.

As a Case Manager your responsibilities will include:

Requirements
