Requirements
  • Community Health Worker certification, or the ability to obtain certification within the first six months of employment.
  • Certified Application Counselor certification, or the ability to obtain certification within the first six months of employment.
  • Strong interpersonal and communication skills and the ability to work effectively with clients in a diverse community.
  • Knowledge of community agencies and resources.
  • Working knowledge of Microsoft Office programs, including Word, Excel, PowerPoint, and Outlook, and Electronic Health Records.
  • High School Diploma or GED.
  • At least one year of applicable experience.
  • A valid Texas driver's license.
  • Ability to meet the physical demands of the position, including frequent medium lifting, bending, keying, reaching, standing, walking, carrying, pushing, and pulling, as well as occasional kneeling and overhead reaching.
Responsibilities
  • Assist clients across home, community, and clinic settings.
  • Educate clients about program benefits and their health impact.
  • Assess non-medical factors affecting health, including insurance status, food, utilities, housing, education, employment, finances, transportation, safety, clothing, child care, and social or emotional health.
  • Coordinate client referrals to address identified needs.
  • Develop personalized support plans.
  • Provide client advocacy.
  • Help clients overcome barriers to accessing medical, behavioral health, and dental care.
  • Support proper utilization of health center services.
  • Process applications and renewals for local, state, and federal programs and resources, including Medicaid, CHIP, SNAP, TANF, Health Center Grant Programs, Sliding Fee Scale, Primary Health Care, Prescription Assistance Programs, Affordable Care Act coverage, and Medicare or Medicare Advantage plans.
  • Document all encounters, assessments, and outcomes.
  • Maintain detailed electronic client records.
  • Complete monthly activity reports.
  • Track client compliance with plan objectives.
  • Coordinate and/or conduct health education presentations.
  • Participate in health center outreach activities, including health promotion events, community clinics, health and resource fairs, food distributions, and community events.
  • Travel to various community locations.
  • Promote awareness of health center services and available assistance programs.
  • Adhere to appearance, safety, business conduct, training, and regulatory compliance standards.
  • Perform other related duties as assigned by the Program Manager and Director of Grants Management and Community Programs.
