Requirements
  • A bachelor's degree in a health-related field of counseling, psychology, social work, or other behavioral health field is required.
  • Candidates with a bachelor's degree in criminal justice must have fifteen college-level semester hours of coursework in behavioral health.
  • A valid Tennessee driver's license is required.
  • An acceptable Motor Vehicle Report is required.
  • An acceptable criminal background investigation is required.
  • Personal automobile insurance is required.
Responsibilities
  • Provide comprehensive care management services to children and youth with severe emotional disorders and their families and/or to adults with severe and persistent mental illness.
  • Engage with consumers monthly to assess needs, develop and address care plan goals, and coordinate care.
  • Complete visits with consumers in the community, including homes, schools, and courts, to evaluate needs and connect with individuals where they are.
  • Address mental health, physical health, social determinants of health, gaps in care, and emergency department and inpatient utilization.
  • Use a side-by-side approach with consumers to evaluate and address barriers to engagement in treatment.
  • Assist with referrals and linkage to community resources and advocate for consumers when necessary.
  • Collaborate with consumers' support systems and identify or increase supports when needed.
  • Follow training handouts and policies to ensure consumer care is delivered with quality.
  • Ensure timely and comprehensive documentation according to agency policies and procedures.
  • Meet productivity standards indicated in policies and procedures.
  • Participate in daily team meetings, share insight, offer solutions, develop action plans, and follow up on plans developed in meetings.
  • Participate in monthly supervision, discuss caseload, engage in professional development discussions, and receive feedback.
  • Develop and maintain collaborative relationships across departments and with community partners.
