Requirements
  • Some college credits from an accredited college or university, or at least two years of experience working with homeless individuals.
  • Ability to work with clients from diverse cultural, ethnic, and socioeconomic backgrounds.
  • Ability to work with homeless clients in the field.
  • Ability to communicate effectively, verbally and in writing, with collaborative entities.
  • Working knowledge of Microsoft Word, Microsoft Excel, and the Internet.
  • Experience working with homeless persons with co-occurring mental health, physical health, developmental disability, and substance use issues.
  • Ability to report legal issues in accordance with applicable law, including Tarasoff concerns, child abuse, vulnerable adult abuse, danger to self or others, and grave disability.
  • Professional experience working within a multidisciplinary team.
  • If in recovery, at least three years of being drug- and alcohol-free is required.
  • Ability to produce verification of employment eligibility and pass a background clearance.
  • Tuberculosis test required within the specified pre- or post-hire period and renewed annually.
  • Current CPR and First Aid certification required within 30 days of employment.
  • Valid driver's license and auto insurance required.
  • Knowledge of evidence-based practices and ability to apply them in the field.
  • Ability to work evenings, overtime, weekends, or holidays.
  • Ability to maintain appropriate professional boundaries and comply with the agency Code of Ethics, HOPICS Core Values, HIPAA, CFR 42, and other funding requirements.
  • Ability to work effectively under pressure and in unpredictable, crisis-oriented, distracting, and changing environments.
  • Ability to perform the physical requirements of sitting, standing, typing, writing, walking, driving, and carrying up to 25 pounds.
Responsibilities
  • Conduct immediate-needs assessments with adult consumers and gather information for comprehensive treatment plans.
  • Collaborate with internal and external entities as part of a multidisciplinary team.
  • Provide field assistance and establish client treatment plans aligned with clinical assessments from therapists, nurses, and substance use counselors.
  • Help consumers resolve or cope with mental, emotional, medical, and substance-related issues contributing to homelessness.
  • Document services, notes, treatment plans, and outcomes in accordance with E6 program guidelines using the Homeless Management Information System.
  • Provide case management, immediate-needs solutions, benefits linkages, housing supports, mental and physical health and substance use program linkages, food support, hygiene support, and employment or school resource linkages.
  • Participate in required meetings, trainings, collaborations, and multidisciplinary team meetings; discuss, educate, present, or evaluate presenting issues and progress.
  • Coordinate program activities to meet contractual agreements, laws, regulations, and funding requirements with the E6 Program Manager.
  • Attend weekly individual and group supervision and other meetings, training sessions, and seminars as needed.
  • Conduct all interventions and treatments in the field, including assigned mandatory program, division, and offsite meetings and trainings.
  • Coordinate transportation and transport E6 staff and participants using the agency van as needed.
  • Maintain documentation of homeless status for intake workers and clients and retain copies in client files.
  • Provide HIV/AIDS information, education, and linkages to testing, housing, and services.
  • Enter activity into the Homeless Management Information System regularly.
  • Assist with collecting and entering information for Los Angeles Homeless Services Authority reports, including Quarterly Performance Reports and Annual Performance Reports.
  • Build rapport with community organizations serving homeless individuals, families, low-income households, and other HOPICS target populations.
  • Inform, educate, and refer homeless individuals and families to HOPICS programs and services.
  • Represent the agency and division professionally at meetings and community events.
  • Maintain regular attendance and client service files and records for compliance and auditing.
  • Coordinate submission or transfer arrangements for client documentation if leaving the position.
  • Perform other assigned duties and provide support for administrative and data-related Information Technology Department functions.
