Four insurers exit Medicaid as margins turn negative.

Four insurers exit Medicaid as margins turn negative. At least four insurers are leaving Medicaid managed care contracts this year and next, a retreat driven by margins that have turned negative as enrollment falls and the remaining members prove costlier to cover. The Medicaid underwriting margin dropped from nearly 3 percent during the public health emergency to negative 1 percent in 2024, below the industry's 1 to 2 percent target, according to a Robert Wood Johnson Foundation analysis. The exits matter for states, which must move members to new plans mid-contract, and for providers, who lose and gain payer relationships as assignments shift. They also raise a question for state Medicaid agencies: whether current rates can sustain a competitive bidding field. Which insurers are exiting Medicaid managed care. According to published reports compiling state and company disclosures: * Centene is leaving two programs. Its Ohana Health Plan exits Hawaii's QUEST managed care program Dec. 31, and Ambetter leaves the Arkansas Health and Opportunity for Me expansion program the same day, reportedly affecting about 70,000 members. * Healthy Blue, a collaboration between Blue Cross and Blue Shield of Louisiana and Elevance Health, ends its Louisiana Medicaid managed care participation Dec. 31, reportedly affecting about 290,000 members. * Elevance Health's Wellpoint exited the District of Columbia managed care program Aug. 1 and is cutting 85 jobs in Washington during the transition. * Baylor Scott & White Health Plan left the Texas Medicaid managed care market in August and cut 321 jobs as it also prepares to exit the individual marketplace. More departures appear likely. On a July earnings call, Elevance executives said they expected additional Medicaid exits over the next 12 to 18 months without naming the markets. Why Medicaid margins collapsed. The core problem is a mismatch between who left the program and how states set rates. Medicaid enrollment peaked at roughly 94.6 million in mid-2023 during pandemic continuous-eligibility protections. After redeterminations resumed, about 25 million people were disenrolled, more than half for administrative or procedural reasons. Enrollment stood at 73.2 million in June 2026, down 5.1 million, or 6 percent, from a year earlier, based on CMS monthly data compiled by KFF. People who left tended to be healthier, which raised the average acuity of those remaining while state capitation rates lagged behind. Medical loss ratios reflect the squeeze. They averaged 90.9 percent in 2024 and rose to 91.2 percent in an analyzed subgroup of states, with Idaho and Minnesota exceeding 100 percent in 2025 and Colorado and Maryland above 95 percent. What insurers are telling investors. Elevance has maintained a full-year 2026 Medicaid operating margin forecast of about negative 1.75 percent, describing 2026 as the trough year. Reporting second-quarter results in July, the company said rate updates came in stronger than expected while cost pressure concentrated in behavioral health, applied behavior analysis therapy, specialty pharmacy, outpatient surgery and emergency department use. Notably, executives attributed that pressure to higher utilization among continuing members rather than a fresh jump in acuity. Chief Financial Officer Mark Kaye said the company was "not seeing a new stepwise acuity reset." Health Benefits Officer Felicia Norwood said Medicaid remains "a very core part of our portfolio," a signal that selective exits are not a wholesale withdrawal. Elevance's second-quarter net income fell 16.6 percent to $1.4 billion, though it raised full-year guidance to adjusted earnings of at least $27 per share. Policy changes ahead. Further enrollment decline looks likely. A national Medicaid work requirement for the expansion population takes effect in 2027 under the budget reconciliation law, and more frequent redeterminations will add administrative churn. Urban Institute analysis cited by the Robert Wood Johnson Foundation projects that 4.6 million to 5.2 million adults in the expansion population could lose coverage. If departures again skew toward healthier members, the acuity problem compounds rather than resolves. What it means for states and providers. State Medicaid agencies face a narrower bidding field precisely as they absorb federal cuts, which weakens their leverage in rate negotiations. Agencies that hold rates down may find fewer bidders at the next procurement. For providers, each exit means renegotiating with whichever plans absorb the members, and transitions carry documented risks of coverage gaps and care disruption. For insurers remaining in Medicaid managed care, the opportunity is market share in states where competitors have left, assuming rates eventually catch up to the acuity they are now covering.

Elevance Health (NYSE: ELV) raises guidance as revenue climbs 0.7%.

Elevance Health (NYSE: ELV) raises guidance as revenue climbs 0.7%. What happened. Elevance Health, Inc. (NYSE: ELV) reported third-quarter 2026 operating revenue of $50.4 billion and raised full-year 2026 guidance. It raised FY 2026 diluted EPS guidance to at least $22.00, adjusted diluted EPS guidance to at least $29.00 and operating cash flow guidance to at least $6.5 billion. Operating revenue rose 0.7% from a year earlier. Diluted EPS was $6.27 and adjusted diluted EPS was $6.99. Quarterly operating cash flow was $1.3 billion, up $0.1 billion year over year. Carelon operating revenue reached $19.3 billion, and Carelon operating gain rose 17% to $0.9 billion. Health Benefits operating revenue was $43.4 billion, but operating gain fell to $0.4 billion from $0.6 billion. The benefit expense ratio was 90.6%, down 70 basis points from a year earlier. The company said stronger performance in Medicare Advantage and Individual ACA helped. It repurchased 0.8 million shares for $330 million and had about $5.0 billion of board-approved share repurchase authorization left. OptimistFi's case is that Elevance turns scale into underwriting earnings only if it keeps medical costs in check and converts earnings into cash. This filing helps that case on cash flow, because the company raised FY 2026 operating cash flow guidance to at least $6.5 billion. It also shows a split in the business, with Carelon improving and Health Benefits weakening. Operating revenue increased by $353 million from the prior-year quarter, but operating gain was unchanged at $1.3 billion. The adjusted operating expense ratio rose 90 basis points to 11.3%, which limits the margin story. The main concern is that Health Benefits operating gain fell to $0.4 billion from $0.6 billion, even with the lower benefit expense ratio. What's next. The conference call was set for today at 8:30 a.m. EDT. The fourth quarter 2026 dividend is payable on December 18, 2026 to shareholders of record at the close of business on December 4, 2026. The replay will be available until the end of the day on November 20, 2026. Investors will look for signs that the higher guidance comes with better margins and steady cash flow. More margin recovery would strengthen the case. Another flat operating-gain quarter would weaken it. From optimistfi perps.
