About the job

Requirements
  • Strong experience with a full modern tech stack for consumer-facing, microservice-based web applications, including modern JavaScript and TypeScript, a component framework such as React, REST API design, object-oriented and functional design, and relational databases.
  • Strong software architecture skills and the ability to design systems that remain reliable and maintainable as scope grows.
  • Proficiency in Python and at least one systems-level language such as C#/.NET or Java; strong SQL skills, including schema design and query optimization.
  • Practical experience with cloud infrastructure, preferably Azure, containerization, infrastructure-as-code, and continuous integration and continuous delivery.
  • Demonstrated effectiveness with AI-aided development tools and agentic workflows, with concrete examples of improving design quality and delivery speed, and judgment about when to keep a human in the loop.
  • Depth in performance and reliability work, including profiling, load testing, caching, latency budgets, and root-cause analysis on live systems.
  • Fluency with automated testing strategy across unit, integration, and end-to-end testing, as well as code review, type safety, and static analysis practices.
  • Working knowledge of application security fundamentals, including authentication and authorization, OWASP-class vulnerabilities, secrets management, and safe handling of sensitive data.
  • Ability to communicate complex technical tradeoffs clearly to engineers and non-technical stakeholders.
  • Sound judgment for ambiguous, high-stakes problems, with a bias toward pragmatic, well-reasoned solutions over gold-plating.
  • Bachelor’s degree in Computer Science, Engineering, or a related field, or equivalent practical experience.
  • 10+ years of professional software engineering experience, including significant hands-on work building and operating production web applications end to end.
  • Demonstrated track record of owning complex technical systems end to end, from design through production operation.
Responsibilities
  • Serve as the senior-most technical voice on full-stack engineering decisions, partnering with the Head of Engineering & AI on strategy and roadmap.
  • Set standards for AI-aided and agentic development, establishing patterns, guardrails, and review practices that accelerate work without compromising design, and drive effective usage across the team.
  • Lead design reviews and architectural decisions for cross-team initiatives, balancing speed, maintainability, and long-term scalability.
  • Identify technical risks in scalability, security, data integrity, or performance and drive them to resolution.
  • Design and own architecture for patient- and clinician-facing applications and the services, APIs, and data models behind them.
  • Set standards for API and service design, including boundaries, versioning, backward compatibility, contract documentation, and authentication and authorization patterns.
  • Drive front-end architecture and performance, including component structure, state management, accessibility, and interaction latency on patient- and clinician-facing hardware.
  • Improve delivery infrastructure, including automated testing strategy, CI/CD, infrastructure-as-code, environment and configuration management, and safe, repeatable deployments.
  • Drive observability across the stack through structured logging, metrics, tracing, and alerting to catch latency, error-rate, and data-quality regressions before they reach patients or clinicians.
  • Build security and privacy into designs through secure coding practices, secrets and access management, dependency risk management, auditability, and protection of protected health information in a regulated environment.
  • Profile and optimize query plans, hot paths, payload sizes, chatty interfaces, and cloud spend, and teach the team to measure before rewriting.
  • Model excellent practices with AI coding agents and assistants by decomposing work, supplying context, specifying intent, and iterating toward sound designs.
  • Review AI-generated code to the same standards as other code, including architectural fit, test coverage, readability, performance, and security, and make those review standards explicit and teachable.
  • Establish and evolve the team’s AI-aided workflow, including tooling, repository conventions and context files, specification and prompting patterns, and guardrails for sensitive code and data.
  • Demonstrate measurable leverage from AI-aided practices and coach engineers on where they are useful and how to evaluate their impact.
  • Mentor engineers through code review, design feedback, and hands-on pairing.
  • Work with global, multicultural teams within a follow-the-sun support and development model.
  • Partner with Product, Clinical, and Hardware teams to translate healthcare and operational requirements into robust technical designs.
  • Represent engineering in cross-functional planning, providing realistic estimates and surfacing tradeoffs early.
  • Perform other related responsibilities as assigned.
Desired Qualifications
  • Experience with healthcare, medical device, or other highly regulated data environments.
  • Exposure to event-driven architecture, background job processing, or real-time communication such as WebSockets or WebRTC.
  • A Master’s degree.

About the company

OnMed provides hybrid-care health technology by licensing self-contained Care Stations. The Care Station is an 8' x 11' kiosk, a Clinic-in-a-Box that enables real-time virtual consultations with licensed clinicians for non-emergency conditions, and it includes tools to measure vitals, high-definition cameras for exams, and an automated medication vault. It differentiates itself with an integrated, robotically automated vault that dispenses hundreds of common medications, along with safety features like locking doors, privacy glass, antimicrobial surfaces, HEPA filtration, and UV-C sanitation. The goal is to expand healthcare access and reduce wait times by deploying these kiosks to deliver immediate care and prescription access where needed.

  • Jackson's Miami deployment expands credibility with safety-net health systems and community partners.
  • Rural Health Transformation applications named OnMed in Virginia and Nebraska, widening 2026 demand.

What critics are saying

  • OnMed depends on payer and government sponsorship; contract renewals define revenue timing.
  • Virtual care kiosks face HIPAA, licensing, and pharmacy-regulation friction across every state.
  • If RHTP contracts stall, OnMed lacks a defensible path to national scale.

What makes OnMed unique

  • OnMed's CareStation combines live clinicians, diagnostics, and dispensing in one 8×10 room.
  • Jackson Health, Triple-S, and Nevada Corrections validate use cases across public systems.
  • Thirty-six active CareStations across 20 states and territories prove deployability beyond pilots.

Jackson Health System deploys OnMed CareStation with Salvation Army in Miami neighbourhood

OnMed has partnered with Jackson Health System and The Salvation Army to deploy a CareStation in Miami's Allapattah neighbourhood. The technology-enabled station, located at 1907 NW 38th St., allows residents to access licensed clinicians without appointments in a private setting. The CareStation offers walk-in primary and urgent care, enabling patients to receive evaluations, diagnoses, treatment plans, and electronic prescriptions during a single visit. The AI-supported environment includes integrated diagnostic tools. Jackson Health System, Miami's only public health system, sponsors the CareStation as part of efforts to extend care beyond traditional hospital settings. The partnership aims to address logistical and economic barriers that often delay treatment for Allapattah residents, whilst creating additional entry points into Jackson's broader healthcare network. The initiative reflects growing efforts by US health systems to combat workforce shortages and emergency room strain through community-based care delivery.

OnMed deploys clinic-in-a-box healthcare stations across three Nevada prisons to cut costs and expand inmate medical access

OnMed has been awarded a state contract with the Nevada Department of Corrections to deploy its CareStation technology across three correctional facilities: High Desert State Prison, Lovelock Correctional Center and Northern Nevada Correctional Center. The deployment was supported by Governor Joe Lombardo through appropriations legislation. The 8×10 foot CareStation connects patients with licensed clinicians via a 55-inch high-definition screen, integrating diagnostic tools including digital stethoscopes and pulse oximeters. The unit requires only an electrical outlet and can be operational within 45 days, resolving non-emergent care on-site in approximately 20 minutes. The system aims to reduce costly off-site medical transports, decrease overtime costs and provide faster treatment. Each visit can result in diagnosis, care plans and e-prescriptions in 240 languages.
