Requirements
  • Two years of clinical experience in a health care setting is desired, with long-term care or outpatient experience preferred.
  • Completion of a dietetic internship.
  • A Bachelor of Arts or Bachelor of Science in dietetics from an Academy of Nutrition and Dietetics-accredited academic institution is required.
  • Current registration or registry eligibility.
  • Ability to effectively assess residents’ or participants’ nutritional needs and establish nutritional care plans.
  • Ability to work effectively in a collaborative environment.
  • Ability to communicate effectively with participants, residents, and families from diverse backgrounds, including those with physical, sensory, and mental impairments.
  • Excellent oral and written communication skills.
  • Proficiency in Microsoft Office and ability to learn new software rapidly.
Responsibilities
  • Evaluate, plan, and clinically assess the nutritional status of residents and participants at long-term care facilities or sites.
  • Coordinate all phases of nutrition care, including nutrition assessment, care planning, monitoring, education, and counseling in a variety of settings.
  • Perform duties in compliance with licensing, accrediting, and regulatory agencies.
  • Contribute to the interdisciplinary team in establishing and implementing the interdisciplinary care plan.
  • Advise the diet technician and home health aides regarding food service operations in the day center.
  • Attend daily team meetings and initial care planning and reassessment meetings.
  • Complete assessments semi-annually and when there is a significant change in status.
  • Actively participate as a member of the tissue trauma team.
  • Demonstrate knowledge of the PACE/D&TC Emergency Preparedness Plan and verbalize the role during an event.
  • Attend and actively participate in required meetings and training sessions.
  • Comply with established policies, procedures, codes of conduct, and health and safety requirements.
  • Practice and teach safety awareness, and identify, report, or correct potential safety or environmental issues.
  • Understand HIPAA and other compliance and regulatory requirements associated with the role, including completing required training or certifications.
  • Perform related duties as assigned.
