Requirements
  • Registered Nurse degree.
  • Three to five years of job-related experience or an equivalent combination of education and experience.
  • Registration with the College of Nurses of Ontario.
  • Proven computer skills, including email, word processing, and work scheduling systems.
  • Thorough knowledge of case management skills and nursing processes and practices.
  • A vehicle is required and regular travel is required.
  • Clear background and Vulnerable Sector Check.
  • The position requires English verbal and written communication skills.
Responsibilities
  • Provide coordination, supervision, and coaching to nurses in the field.
  • Develop and maintain an effective care plan for Community Health Services, independently or in cooperation with Ontario Health atHome.
  • Keep abreast of policies outlined in the Regulated Health Professions Act and Community Health Services best practices, and monitor employee performance.
  • Promote and market Community Health Services and maintain positive public relations.
  • Provide active guidance for workplace health and safety.
  • Perform other duties as required.

  • Leadership and people management skills are an asset.
  • French language skills are an asset.
