Requirements
  • At least one year of experience as a registered nurse in acute care hospital, oncology, long-term care, hospice, or home health; school internships and clinical hours may be considered in individual cases.
  • A current license to practice nursing in the state of employment is required.
  • Current CPR certification is required.
  • A current automobile insurance policy and valid driver’s license are required.
  • Knowledge of terminally ill patients and their families, hospice services, the roles of hospice disciplines, community resources, general nursing practice, managed care principles, and regulatory guidelines including Medicare, Medicaid, and ACHC is required.
  • The ability to perform patient assessments, document clinical information, communicate effectively, support patients and families during crisis, operate nursing assessment tools, computers, facsimile equipment, copier, and cell phone or beeper, and maintain transportation for work is required.
Responsibilities
  • Serve as the primary point of contact for hospice patients and families throughout the hospice journey.
  • Deliver and document skilled nursing care according to individualized plans of care in patients’ homes, assisted living facilities, or other settings.
  • Perform initial, ongoing, comprehensive, and reassessment screenings, including in-person and telephonic registered nurse assessments.
  • Collaborate with physicians, licensed practical nurses, certified nursing assistants, social workers, chaplains, nurse practitioners, and other interdisciplinary team members to develop and update individualized care plans.
  • Provide emotional, physical, and spiritual support, education, comfort, bereavement guidance, and practical training to patients, families, and caregivers.
  • Teach patients, families, and caregivers about disease processes, self-care, end-of-life care, medications, nutrition, dietary needs, hospice philosophy and services, symptom and pain management, and end-of-life processes and expectations.
  • Deliver skilled nursing interventions and symptom management aligned with goals of care.
  • Develop and update individualized plans of care, including visit frequency and risk identification.
  • Communicate changes in condition and signs or symptoms of decline to appropriate providers, including nurse practitioners, primary care providers, and hospice physicians.
  • Support continuity of care by recognizing and supporting home health, palliative, and hospice referrals and transitions in levels of care.
  • Maintain timely and accurate clinical records, including screening and assessment findings, physician orders, progress notes, and care plans.
  • Participate in quality improvement, scheduling initiatives, interdisciplinary group activities, team collaboration, on-call services, and patient support activities.
