About the job
- At least 1 year of experience in case management or social services.
- Knowledge of available financial and social service resources in southern New Jersey.
- A bachelor's degree in Social Work, Counseling, Psychology, or a related field is required.
- Provide comprehensive HIV case management and prevention services to individuals living with HIV and those at high risk for HIV infection.
- Manage a caseload of up to 65 HIV-positive clients, including emergent case management for EIP patients.
- Conduct assessments and develop and monitor individualized care plans in accordance with AACO and New Jersey Department of Health standards.
- Link clients to appropriate medical, behavioral, and support services through referrals and care coordination while addressing immediate and ongoing needs.
- Experience in the field of HIV.
- AACO or New Jersey HIV Case Management Certification.
- Excellent interpersonal skills.
