- A nursing degree from an accredited college or university is required.
- A current unrestricted license as a Licensed Practical Nurse (LPN) in the practicing state is required.
- One or more years of nursing experience is preferred.
- Ability to demonstrate compassion when working with residents.
- Direct the daily activities of certified nursing assistants and resident care associates in accordance with federal, state, and local regulations and established company policies and procedures.
- Follow the nursing model to promote holistic care for residents.
- Ensure that policies and procedures are followed by assigned nursing personnel.
- Participate in resident admissions, discharges, and transfers as required.
- Assess changes in residents’ status, notify the physician and resident’s family or representative, and document accordingly.
- Transcribe physician orders to the medical record and carry out orders as written.
- Prepare and administer medications according to physicians’ orders and observe for adverse effects.
- Perform wound treatments according to physicians’ orders, observe for changes, and document accordingly.
- Schedule resident follow-up appointments and transportation as indicated.
- Provide nursing leadership to assigned nursing personnel.
- Provide direct care skills including colostomy changes, tube feedings, wound care, suctioning, and intravenous administration in accordance with current policies and procedures.
- Perform rounds to ensure resident needs are met and personnel perform their assigned duties.
- Collaborate with interdisciplinary team members to ensure residents’ needs are holistically met.
- Report incidents or unusual occurrences to the community’s leadership team.
- Ensure adequate stock of medications, supplies, and equipment, and notify appropriate personnel of needs.
- Initiate, review, and update care plans as required.
- Participate in the survey process during inspection surveys.
- Participate in quality assurance and performance improvement activities as needed or assigned.
- Ensure infection control principles are followed and help correct deficiencies in practice.
- Perform other duties as assigned.
- Comply with all policies and procedures and updates.
