Responsibilities
- Report to the nurse manager or nurse supervisor at Premier HealthCare and meet weekly for supervision to review duties and responsibilities.
- Manage day-to-day operations of the nursing station and ensure workflows and policies are implemented and followed as directed.
- Assume nurse supervisor duties when needed and provide coverage for and assistance with PCMH and other grants.
- Supervise nursing staff, including medical assistants and medical secretaries where applicable.
- Use the designated PHC greeting according to policy.
- Ensure each on-site provider has an assigned LPN or medical assistant to assist with daily workflow.
- Document activities in the electronic medical record system once trained and the system is operational.
- Serve as a Patient-Centered Medical Home care team member, coordinate and manage the high-risk population, lead daily huddles, and communicate with care team members.
- Keep provider schedules current and prepare for daily huddles by reviewing high-risk groups, pending referrals, required labs or testing, needed immunizations, and reasons for visits.
- Participate in monthly care team meetings and provide patient updates as needed.
- Check exam rooms and ensure computers are logged in and functioning before patient visits; report issues to the nurse supervisor or designee.
- Escort patients to and from exam rooms, prepare them for clinician visits, and verify patient names, dates of birth, and demographic information.
- Verify and update each patient's assigned primary care provider in the electronic health record as needed.
- Accurately obtain and document assigned vital signs, height, weight, and body mass index in the patient's medical record.
- Accurately document pertinent information in electronic health records, including reason for visit, medical history, medication updates, allergies, screenings, and PCMH-related information.
- Complete the PCMH tab in the electronic health record with nursing questions, provide education sheets to patients in high-risk groups, and reinforce provider-set goals.
- Obtain an updated medication list at every visit, notify the provider of medication changes, and complete the medication reconciliation record when there are no changes.
- Collect forms or documents requiring provider review and signature and inform the provider.
- Prepare medical records for clinicians, including laboratory and diagnostic reports and stamped progress notes as needed; obtain pending imaging or test results completed since the last visit from the patient or caregiver.
- Perform phlebotomy on adult and pediatric patients and collect other specimens as ordered by clinicians in compliance with Department of Health and Clinical Laboratory Improvement Amendments protocols; perform electrocardiograms and other office procedures as ordered.
- Complete vaccinations, medication standing orders, purified protein derivative placements, and other tasks within scope of practice as ordered by the provider.
- Meet with patients to complete intake packets or updates and supporting documentation, and enter information in the electronic health record as instructed.
- Assist with patient examinations as directed by the clinician or nurse, and assist with therapeutic interventions to ensure patient and staff safety using techniques learned during pre-service, orientation, and ongoing training.
- Assist with checkout as needed by printing patient plans, referrals, or prescriptions, and refer patients to the front desk to schedule return visits and internal referrals.
- Enter prescription and form requests in the designated database, complete associated documentation such as prescription fax forms, and update the electronic health record.
- Obtain prior authorization for non-covered medications or feeding supplies as needed.
- Notify patients of laboratory results as directed by the provider and document the communication in the electronic health record.
- Maintain cleanliness and organization of treatment rooms and other assigned practice areas; remove filled sharps disposal units and other biohazardous waste for transporter removal and replace units as needed.
- Promote continuity of care and serve as a liaison among Premier HealthCare, patients, families, caregivers, and other agencies involved in patient care.
- Participate in telephone triage to ensure requests are appropriately made and prioritized and have supporting documents before the first visit.
- Document hospital and emergency room follow-up in the designated electronic health record template, obtain hospital discharge summaries before visits, review with providers whether visits were avoidable, document findings in patient charts, and follow directed workflows.
- Participate in quality improvement activities as directed by the Practice Administrator or designee.
- Resupply and maintain inventory as assigned by the nurse supervisor or manager.
- Comply with Occupational Safety and Health Administration and Clinical Laboratory Improvement Amendments regulations while performing clinical and related functions.
- Report safety and health hazards to appropriate personnel promptly.
- Assist with emergencies as directed by the charge nurse, designee, or clinician.
- Comply with New York State Department of Health regulations and protocols in the PHC Policy and Procedures Manual while performing clinical and related activities.
- Check refrigerator and freezer temperatures daily.
- Check and sign off on daily emergency equipment checks and check the emergency cart monthly; ensure the crash cart and emergency medications are current.
- Check vaccine inventory and expiration dates weekly or as directed, and ensure vaccines are labeled correctly and vaccine information statements are current.
- Maintain competencies through on-site training, continuing education programs, and review of reference materials.
- Endorse and receive information before and after lunch or breaks.
- Review and respond to emails on the day they are received.
- Complete assigned tasks daily.
- Perform other duties as assigned by the supervisor or designee.
Desired Qualifications
- Experience with electronic health record and scheduling systems is preferred.
- Experience in an outpatient medical facility is preferred.
- Experience with individuals with intellectual or developmental disabilities is preferred.
