Requirements

Responsibilities
  • Report to the nurse manager or nurse supervisor and meet weekly for supervision to review duties and responsibilities.
  • Manage the day-to-day operations of the nursing station and ensure workflows and policies are implemented and followed.
  • Assume nurse-supervisor duties when needed and provide coverage and assistance for Patient-Centered Medical Home and other grants.
  • Supervise nursing staff, including medical assistants and medical secretaries when applicable to the site.
  • Ensure each provider has an assigned Licensed Practical Nurse or medical assistant to support daily workflow.
  • Document activities in the electronic medical record system after training and system implementation.
  • Coordinate and manage high-risk populations as a Patient-Centered Medical Home care-team member.
  • Lead and participate in daily care-team huddles, reviewing high-risk groups, pending referrals, laboratory or testing needs, immunizations, and visit reasons.
  • Participate in monthly care-team meetings and provide patient updates.
  • Check examination rooms and verify that computers are operational before patient visits.
  • Escort patients to and from examination rooms and prepare them for clinician visits, verifying names, dates of birth, and demographics.
  • Verify and update assigned primary care providers in the electronic health record.
  • Obtain and document vital signs, height, weight, and body mass index accurately.
  • Document pertinent patient information in electronic health records, including visit reasons, medical history, medication updates, allergies, screenings, and Patient-Centered Medical Home information.
  • Complete the Patient-Centered Medical Home tab in the electronic health record, provide education sheets to high-risk patients, and reinforce provider-established goals.
  • Obtain updated medication lists at every visit, notify providers of changes, and complete medication reconciliation documentation.
  • Collect patient forms and documents requiring provider review and signatures.
  • Prepare medical records for clinicians, including laboratory and diagnostic reports and progress notes.
  • Obtain pending imaging or testing completed after the last visit from patients or caregivers.
  • Perform phlebotomy for adult and pediatric patients and collect other specimens according to clinician orders and Department of Health and Clinical Laboratory Improvement Amendments protocols.
  • Perform electrocardiograms and other office procedures according to clinician orders.
  • Administer vaccinations, medication standing orders, purified protein derivative placements, and other tasks within the scope of practice as ordered by providers.
  • Complete patient intake packets and updates and enter supporting information in the electronic health record.
  • Assist clinicians and nurses with patient examinations and therapeutic interventions that support patient and staff safety.
  • Assist with checkout by printing patient plans, referrals, and prescriptions and directing patients to schedule return visits and internal referrals.
  • Enter prescription and form requests into the designated database and complete associated documentation.
  • Obtain prior authorizations for non-covered medications or feeding supplies as needed.
  • Notify patients of laboratory results as directed by providers and document notifications in the electronic health record.
  • Maintain cleanliness and organization of treatment rooms and practice areas, and manage sharps and biohazardous waste for removal.
  • Promote continuity of care and serve as a liaison among the healthcare center, patients, families, caregivers, and other agencies.
  • Participate in telephone triage and ensure requests are appropriately documented, prioritized, and supported before the first visit.
  • Document hospital and emergency-room follow-up in the designated electronic health record template and obtain hospital discharge summaries before visits.
  • Review hospital follow-up with providers, assess whether visits were avoidable, and document findings in patient charts.
  • Participate in quality-improvement activities as directed.
  • Resupply and inventory materials as assigned.
  • Comply with Occupational Safety and Health Administration, Clinical Laboratory Improvement Amendments, and New York State Department of Health regulations and protocols.
  • Report safety and health hazards promptly.
  • Assist with emergencies as directed by the charge nurse, designee, or clinician.
  • Check refrigerator and freezer temperatures daily.
  • Check emergency equipment daily and the emergency cart monthly, and ensure crash-cart equipment and emergency medications remain current.
  • Check vaccine inventory and expiration dates weekly or as directed, and ensure vaccine labels and Vaccine Information Statements are current.
  • Maintain competencies through on-site training, continuing education, and reference-material review.
  • Exchange information before and after lunch or breaks.
  • Review and respond to emails on the day received.
  • Complete assigned daily tasks and perform other duties assigned by the supervisor or designee.
Desired Qualifications
  • Experience with various electronic health record and scheduling systems is preferred.
  • Experience in an outpatient medical facility is preferred.
  • Experience working with individuals with intellectual and/or developmental disabilities is preferred.
