About the job
- Graduation from an accredited registered nursing school and qualification as a Registered Nurse.
- A current Georgia Registered Nurse license, Nursing Compact or multistate license.
- Two years of previous clinical experience.
- The ability to interact with patients, families, physicians, and hospital staff.
- The ability to type accurately at approximately 30 words per minute.
- Work with neonate, adolescent, adult, and geriatric patients, families, medical staff, and community agencies to determine the appropriate care setting and appropriateness of hospitalization.
- Assess post-hospital care needs.
- Provide support to address social drivers of health.
- A Bachelor of Science in Nursing.
- Three years of experience in case management, social services, or a related field.
- Certified Case Manager certification.
- Excellent oral and written communication skills.
