Clinical Team Leader – Connected Care
Connected Care is seeking an experienced and passionate Clinical Team Leader to lead the day-to-day operations of our clinical workforce across Northwest Tasmania.

About Connected Care

Connected Care is a statewide, integrated mental health service delivered by Wellways in partnership with Stride Mental Health and Clarity Health Care and commissioned by Primary Health Tasmania (PHT).

Supporting adults aged 18 years and over, Connected Care provides free mental health care and support through a ‘no wrong door’ approach and stepped-care model. Our centralised statewide after-hours service provides additional access to support outside core service hours.

About you

As Clinical Team Leader, you will provide visible, accessible and hands-on leadership to the Northwest Tasmania clinical team.

You will oversee the day-to-day coordination of the clinical workforce, providing practice guidance, supporting clinical decision making and ensuring service delivery remains safe, responsive and aligned with Connected Care's model of care.

Working closely with the Service Manager and fellow Team Leaders, you will translate organisational priorities into effective daily practice, support workforce capability and respond to operational and clinical matters as they arise.

You will play an important role in bringing together our clinical, peer and psychosocial teams, ensuring they work collaboratively with participants and their wider support networks.

Importantly, you will remain connected to clinical practice through your own caseload, providing evidence-based therapeutic interventions while supporting the capability and confidence of your team.

In this role, you will:

• Provide day-to-day leadership and operational oversight of the clinical workforce

• Coordinate team workflows, participant allocation, rostering and workforce coverage

• Provide clinical practice guidance and support sound clinical decision-making

• Facilitate complex case discussions and clinical case reviews

• Support clinicians to manage complex presentations and appropriately escalate clinical risks

• Respond to operational issues and escalate significant incidents, complaints and risks as required

• Support the implementation of organisational initiatives and local service improvements

What you'll bring

You will be a confident and approachable clinical leader who can balance people leadership, operational responsibilities and high-quality clinical practice.

• Relevant tertiary qualification and current registration or accreditation, at the level required for that discipline – refer to position description for detailed list

• Demonstrated experience in mental health or community services

• Experience providing clinical leadership, line management or practice guidance

• Demonstrated ability to coordinate and support multidisciplinary teams

• Experience working with complex mental health presentations

• A strong understanding of recovery-oriented and trauma-informed practice

• A commitment to culturally safe, inclusive and person-centred practice

• Right to work in Australia

• A current Driver Licence and willingness to travel as required

• Satisfactory National Police Record Check

• International Police Check (if applicable)

• Current Working with Vulnerable People/Working with Children Check and NDIS Worker Screening Check

• NDIS Worker Orientation Module Certificate – Quality, Safety and You – free online course
