- Serve as the first contact for non-emergent needs, including medication refills, scheduling, and appointment reminders.
- Anticipate patients’ needs and bridge gaps in care.
- Assist patients with healthcare navigation, including scheduling appointments, coordinating with specialists, and ensuring preventive screenings are completed.
- Review and evaluate in-home device readings in real time during normal business hours.
- Follow established protocols for identifying, communicating, and documenting device trends and associated symptoms in patients’ medical records.
- Escalate concerning readings or symptoms to providers according to established communication protocols.
- Build meaningful rapport and promote trust, open communication, and motivation for positive health changes.
- Collaborate with virtual team members and in-office staff to ensure patients’ needs are met.
