- Educate patients about the frequency and use of their assigned in-home monitoring devices.
- Create a personalized, comprehensive care plan with each patient through phone or video visits.
- Identify and address barriers to patient success.
- Provide education and coaching on patients' chronic conditions.
- Connect with patients frequently to review readings and provide a monthly update to their care plans.
- Serve as the patient's first contact for non-emergency needs, such as medication refills, scheduling, and appointment reminders.
- Anticipate patients' needs and bridge gaps in care.
- Assist patients in navigating healthcare, including scheduling appointments, coordinating with specialists, and ensuring preventative screenings are completed.
- Review and evaluate in-home device readings in real time during normal business hours.
- Follow established protocols to identify, communicate, and document device trends and associated symptoms in the patient's medical record.
- Escalate concerning readings or symptoms to the provider according to the provider's communication protocol.
- Build meaningful rapport and trust through open communication, motivating patients to make positive changes in their health.
- Collaborate with virtual team members and in-office staff to ensure patients' needs are met.
