About the job
- Experience related to patient education, motivational interviewing, and self-management goal setting.
- Experience with outbound patient telecommunications.
- Ability to interact productively with individuals and multidisciplinary teams.
- Ability to travel throughout the assigned region and conduct home visits as needed.
- Knowledge of chronic diseases, especially chronic obstructive pulmonary disease, asthma, diabetes, congestive heart failure, hypertension, and chronic renal failure.
- Proficient computer skills for working efficiently with electronic medical records.
- Licensed Practical Nurse, Licensed Vocational Nurse, or another approved credentialed health professional.
- Active and unrestricted New Mexico or compact-state license.
- Current basic life support certification.
- At least 2 years of experience in a physician's office, clinical setting, or hospital setting.
- Support patients with high-risk health conditions in navigating the healthcare system.
- Interact and collaborate with multidisciplinary care teams, including physicians, nurses, pharmacists, laboratory technologists, social workers, and educators, to support chronic care management and transitions of care.
- Act as a resource for clinic staff.
- Perform nursing duties within the scope of an applicable registered nurse, licensed vocational nurse, or licensed practical nurse license.
- Bilingual English and Spanish ability.
- Strong verbal and written communication skills.
- Solid interpersonal skills.
- Strong organizational and prioritization skills.
