Requirements

  • A valid driver's license and automobile insurance are required for work-related travel.
  • Ability to transport oneself to and from home visits.
  • Ability to occasionally ascend and descend stairs, assist children during activities, move classroom and event furnishings and toys, lift up to 25 pounds, and work in outdoor weather conditions.
  • Compliance with HIPAA regulations.
Responsibilities
  • Recruit and maintain a caseload of up to 50 participants and conduct monthly contact with each participant; serve 100 participants within a program year.
  • Establish and maintain cooperative, trusting relationships with individuals, families, and clinical and community-based organizations.
  • Use caring, non-judgmental language.
  • Share appropriate Healthy Start information and encourage or navigate referrals to and from clinics and community-based organizations.
  • Use varied outreach methods to engage individuals and groups in diverse settings.
  • Complete required assessments, including Healthy Start tools, to understand participants, their families, and their communities.
  • Assist participants and families with creating goals documented in an Individual Family Service Plan, updated quarterly.
  • Provide home-based services to women, men, and infants or children, including health education, breastfeeding and lactation support, doula services, and other case-management activities.
  • Promote father and partner engagement with infants and children.
  • Explain the importance of well-child visits and immunizations, and assist participants with finding a medical home for infants or children and maintaining recommended appointments.
  • Promote ongoing parent-child interactions, including daily reading to infants and children.
  • Screen and refer participants for substance abuse and postpartum depression treatment as indicated.
  • Assist participants with completing a Family Reproductive Plan.
  • Educate prenatal clients about child development and skills that enhance child development, and provide developmental screenings using the ASQ-SE-2 and ASQ-3 screening tools.
  • Educate participants about infant safety, including safe sleep practices.
  • Help identify children with special needs and make necessary referrals.
  • Advocate for and promote culturally and linguistically appropriate services and resources within the Healthy Start program and with community partners.
  • Follow organizational policies, procedures, Healthy Start guidelines, and SGA policies and procedures.
  • Contribute to prevention and integrated service delivery efforts with community providers.
  • Participate in the agency's Continuous Quality Improvement activities.
  • Maintain client family records according to agency and program standards.
  • Compile and enter data into ETO as required by the program and agency.
  • Provide data and information for program reporting.
  • Inform the Healthy Start Team about resources available to community families.
  • Develop a consistent schedule for visiting program participants.
  • Perform additional assigned duties.
Desired Qualifications
  • Bilingual or bicultural proficiency in English and Spanish is preferred but not required.
