Must travel to community positions using a personal vehicle; mileage reimbursement is provided.
About the job
- Must have a reliable personal vehicle and proof of an active New York State driver's license.
- Must hold current New York State certification as a Home Health Aide or be willing to complete training to become certified.
- Must be able to read, write, and speak English.
- Must successfully complete physical capability requirements.
- Assist participants with Activities of Daily Living by providing direct personal care and homemaker services at home and/or at the Day Center.
- Assist with the development, provision, and revision of a participant's Plan of Care.
- Serve and feed participants, assist with meals, and clean up after meal completion.
- Monitor participants' physical or emotional health, note changes, and communicate them to the Nurse and Interdisciplinary Team.
- Follow the Plan of Care by recording, monitoring, and assisting participants with self-administered medications, limited to retrieving and opening containers.
