Requirements
  • Meet the education and licensure requirements for occupational therapists under Vermont professional standards.

Responsibilities
  • Manage a caseload of clients enrolled in the Confluence Transitions program.
  • Conduct weekly individual occupational therapy and group sessions with program participants.
  • Perform assessment, treatment, and discharge planning.
  • Conduct continued assessments and participant check-in sessions.
  • Liaise with and support direct care staff, including mentors and coaches, to improve participant support, the therapeutic process, and the intervention model.
  • Review medical history, observe daily tasks, and test physical, mental, and cognitive capabilities.
  • Create personalized, goal-oriented treatment plans with specific therapeutic activities and exercises.
  • Help clients relearn daily tasks such as dressing, eating, bathing, and cooking.
  • Recommend home, school, and workplace adaptations and safety modifications, including assistive equipment.
  • Educate caregivers and family members on how to support and care for patients safely.
  • Monitor and record patient progress and adjust care plans as necessary.
  • Consult directly with the clinical and Transitions teams and participate in weekly clinical team meetings.
  • Correspond with referring professionals, previous clinicians, and other programs or schools.
  • Support site visits by professionals and potential participants.
  • Participate in marketing events and presentations at conferences.
  • Write and edit publication content, including marketing collateral, websites, and blogs.
  • Provide periodic on-call availability for emergency clinical needs with direct care staff.
