  • An active Florida Registered Nurse license in good standing is required.
  • Knowledge of discharge planning, community resources, and care coordination practices is required.
  • The candidate must pass a Level 2 background screening through the Florida Department of Children and Families and the Agency for Health Care Administration Clearinghouse.
Responsibilities
  • Serve as the primary nursing care coordinator for assigned individuals.
  • Conduct timely follow-up and care coordination related to high utilizers, emergency department visits, hospitalizations, and medical send-outs.
  • Identify medically complex and high-risk individuals requiring enhanced care coordination and intervention.
  • Coordinate primary care, specialty care, psychiatric, pharmacy, and behavioral health appointments.
  • Assist with discharge planning and continuity of care following inpatient detox admissions.
  • Monitor medication continuity, treatment compliance, and follow-up recommendations.
  • Coordinate care with hospitals, healthcare providers, pharmacies, managed care organizations, and community agencies.
  • Assist individuals with accessing insurance benefits, medication assistance programs, transportation services, and community-based resources.
Desired Qualifications
  • Experience in behavioral health, substance use, care coordination, or medical case management is preferred.
  • Strong communication, documentation, and organizational skills are preferred.
